右美托咪定和丙泊酚预处理对大鼠缺血再灌注和肠黏膜屏障完整性的影响

【摘要】:目的:比较右美托咪定(dexmedetomidine,DEX)、丙泊酚(propofol,PRO)预处理对大鼠肠缺血/再灌注损伤(intestinal ischemia-reperfusion injury,II/R)和肠黏膜屏障完整性的影响。方法:采用夹闭大鼠肠系膜上动脉(superior mesenteric artery,SMA)的方法建立肠缺血/再灌注损伤模型的Wistar大鼠40只。随机分为4组:假手术组(Sham组)、II/R组、PRO(10mg/kg)组、DEX(5μg/kg)组,每组10只。药物预处理组于夹闭SMA前20 min,分别经股静脉输注PRO、DEX;于灌注末2 h抽腔静脉血测血清二胺氧化酶(diamine oxidase,DAO)活性,用光镜观察肠黏膜上皮病变并行组织病理损伤评分(Chiu's评分)。结果:1、与Sham组比较,II/R组DAO活性明显增高(P0.05);PRO预处理II/R可降低DAO的含量(34.29±7.33 vs 45.46±8.46;(P0.05);但较Sham组及DEX组升高((P0.05);DEX预处理II/R后能显著降低DAO的含量且较PRO组更有意义,其含量为(23.33±7.06 vs 45.46±8.46;(P0.05),但较Sham组升高(P0.05)。2、II/R组肠绒毛破溃,肠黏膜变薄,其完整性破坏严重。PRO与DEX相比对II/R保护效应较差,两组各参数比较差异仍有统计学意义(P0.05);DEX预处理与II/R组相比,可使肠粘膜损伤降低,很好维护肠粘膜的完整性,且优于PRO。结论:肠缺血/再灌注后,严重的破坏肠黏膜屏障完整性。右美托咪定较丙泊酚预处理能更有效维护肠黏膜的完整性,减少血清DAO的表达,能对大鼠肠缺血/再灌注的肠黏膜起到保护作用,且比丙泊酚组对受损肠道的保护作用更突出。
[Abstract]:Aim: to compare the effects of dexmetomidine (dexmedetomidine,DEX) and propofol (propofol,PRO) preconditioning on intestinal ischemia / reperfusion injury (intestinal ischemia-reperfusion injury,II/R) and intestinal mucosal barrier integrity in rats. Methods: 40 Wistar rats with intestinal ischemia / reperfusion injury were established by clamping superior mesenteric artery (superior mesenteric artery,SMA). They were randomly divided into 4 groups: sham operation group (Sham group) and pro (10mg/kg) group (10 rats in each group). The serum diamine oxidase (diamine oxidase,DAO) activity was measured 20 min, before SMA clipping in the drug preconditioning group by intravenous infusion of PRO,DEX; through femoral vein at the end of 2 hours of perfusion. The intestinal mucosal epithelial lesions were observed with light microscope and the pathological injury score (Chiu's) was evaluated.
