快速康复外科在全膝关节置换术后功能锻炼的应用

【摘要】：目的探讨快速康复外科在膝关节置换术后功能锻炼的应用效果。方法选择2015年4月1日至2016年7月1日,于我院骨科行膝关节置换术的病例49例66膝,随机分为快速康复组(以下简称F组,25例33膝)和传统康复组(以下简称T组,24例33膝)。两组患者性别、年龄、侧别、HSS评分、膝关节主动活动度术前差异均无统计学意义(p0.05)。膝关节置换术后,快速康复组在快速康复外科理念指导下进行功能锻练,训练的开始时间较传统康复有所提前,训练的频率和强度明确说明;传统康复组患者按照传统康复理念进行功能锻练。分别于术后1个月、3个月比较两组膝关节主动活动度、HSS评分。结果膝关节置换术后1个月时,F组膝关节主动活动度为(105.6±8.1)°高于T组的(94.1±8.5)° ( p0.05 ),F 组 HSS 评分为 69.4±4.1 优于 T 组的 59.2±6.4 ( p0.05 );术后3个月时,F组的膝关节主动活动度为(116.3±7.2)°高于T组的(98.5±6.9)°(p0.05 ), F 组 HSS 评分为 80.6±3.0 优于 T 组的 68.3±4.2 ( p0.05 )。结论快速康复外科方法应用于膝关节置换术后功能锻炼,有利于患者加快术后康复。
[Abstract]:Objective to investigate the effect of rapid rehabilitation surgery on functional exercise after knee arthroplasty. Methods from April 1, 2015 to July 1, 2016, 49 cases (66 knees) with knee arthroplasty were randomly divided into two groups: group F (25 cases, 33 knees) and traditional rehabilitation group (group T, 24 cases, 33 knees). There was no significant difference in sex, age, HSS score and active range of knee joint between the two groups (p0.05). After knee arthroplasty, the rapid rehabilitation group underwent functional exercise under the guidance of the concept of rapid rehabilitation surgery, and the training began earlier than the traditional rehabilitation. The frequency and intensity of the training were clearly explained. The patients in the traditional rehabilitation group underwent functional exercise according to the traditional rehabilitation concept. The score of HSS was compared between the two groups at 1 month and 3 months after operation. Results the HSS score of group F was (105.6 卤8.11) 掳higher than that of group T (94.1 卤8.5) 掳at 1 month after knee arthroplasty, and the HSS score of group F was 69.4 卤4.1 掳, and that of group F was 59.2 卤6.4 掳(p0.05) at 3 months after operation, the range of active motion of knee joint in group F was significantly higher than that in group T (P 0.05). It was (116.3 卤7.2) 掳higher than that of T group (98.5 卤6.9) 掳(p0.05), F group: 80.6 卤3.0o vs T group: 68.3 卤4.2 掳, p0.05). Conclusion the application of rapid rehabilitation surgery in functional exercise after knee arthroplasty is helpful to accelerate postoperative rehabilitation.
