自体骨髓间充质干细胞植入联合髓芯减压植骨支撑术治疗早中期股骨头坏死的临床研究

【摘要】：目的:观察自体骨髓间充质干细胞植入联合髓芯减压植骨支撑术治疗早中期股骨头坏死的临床效果。方法:选取2016年3月～2017年3月在我院就诊的80例早中期股骨头坏死患者作为研究对象,采用随机数字表法分为对照组和观察组,每组40例。对照组采用髓芯减压植骨术治疗,观察组采用自体骨髓间充质干细胞植入联合髓芯减压植骨支撑术治疗。比较两组的治疗效果、疼痛程度和Harris关节功能评分。结果:观察组的治疗优良率明显高于对照组,差异有统计学意义,P0.05;治疗前,两组的VAS评分和Harris评分相比较,差异均无统计学意义,P0.05;治疗后,两组的VAS评分均较治疗前明显降低,两组的Harris评分均较治疗前明显升高,且观察组的疼痛程度降低幅度和关节功能改善幅度均优于对照组,差异均有统计学意义,P0.05。结论:自体骨髓间充质干细胞植入联合髓芯减压植骨支撑术治疗早中期股骨头坏死临床效果显著,可有效改善患者的关节功能,减轻疼痛程度,总体疗效相对较高。
[Abstract]:Objective: to observe the clinical effect of autologous bone marrow mesenchymal stem cells implantation combined with core decompression and bone grafting in the treatment of early and middle stage femoral head necrosis. Methods: 80 patients with osteonecrosis of femoral head from March 2016 to March 2017 in our hospital were selected and randomly divided into control group (n = 40) and observation group (n = 40). The control group was treated with bone marrow core decompression and bone grafting, while the observation group was treated with autologous bone marrow mesenchymal stem cells implantation combined with bone marrow core decompression and bone grafting. The therapeutic effect, pain degree and Harris joint function score were compared between the two groups. Results: the excellent and good rate of treatment in the observation group was significantly higher than that in the control group, the difference was statistically significant (P 0.05). Before treatment, there was no significant difference in VAS score and Harris score between the two groups (P 0.05). After treatment, the VAS score of the two groups was significantly lower than that of the control group, and the Harris score of the two groups was significantly higher than that of the control group, and the degree of pain reduction and the improvement of joint function in the observation group were better than those in the control group, and the difference was statistically significant. P0.05. Conclusion: the clinical effect of autologous bone marrow mesenchymal stem cells implantation combined with core decompression and bone grafting in the treatment of femoral head necrosis in the early and middle period is remarkable. It can effectively improve the joint function and alleviate the degree of pain. The overall curative effect is relatively high.
