腰椎后路椎间融合术后患者满意度的临床观察

【摘要】：目的：通过腰椎后路椎间融合术后患者满意度相关因素研究以便提高医疗服务质量,为其他相关研究服务。方法：回顾性地研究并分析2013.2-2014.10期间,于成都中医药大学附属医院骨伤科收集83例腰椎退变致腰腿痛,并在随后接受腰椎后路椎间融合术(PLIF)手术的患者。在术后6个月后对患者进行满意度调查,将患者分成满意组、不满意两组。在术前与术后6个月分别用SF-36量表评估患者主观的生活质量,用JOA量表评估客观的治疗效果,通过患者满意度与各种基线参数(一般资料、症状持续时间、影像学发现、SF-36评分、JOA评分)的比较,从而找出影响患者满意度最主要的术前预测因素。结果：总共81位患者完成了整个研究,其中64(79%)位患者对手术结果感到满意。不满意组在术前SF-36评分上的躯体疼痛、一般健康感知、活力方面与满意组存在显著的差异。在术后6个月,两个组的SF-36评分在所有的8个方面都存在显著的差异,但是两组的JOA评分未见明显差异。结论：腰椎后路椎间融合术后,患者在躯体疼痛、总体健康、活力方面,较低术前生活质量评分与术后满意度正相关。
[Abstract]:Objective: to improve the quality of medical service and other related research by studying the related factors of patient satisfaction after lumbar posterior lumbar interbody fusion. Methods: a retrospective study of 83 cases of lumbar degenerative lumbago and leg pain caused by lumbar degenerative pain and (PLIF) operation was performed in the Department of Orthopaedics and Trauma of Chengdu University of traditional Chinese Medicine from March 2 to October 2014. The patients were investigated 6 months after operation and divided into two groups: satisfactory group and unsatisfactory group. The subjective quality of life (QOL) was evaluated by SF-36 scale before operation and 6 months after operation. Objective therapeutic effect was evaluated by JOA scale. Patients' satisfaction and baseline parameters (general data, duration of symptoms, imaging findings) were evaluated. SF-36 score and JOA score were compared to find out the most important predictors of patient satisfaction. Results: a total of 81 patients completed the study, of which 64 (79%) were satisfied with the results. There were significant differences in body pain, general health perception and vitality between the unsatisfactory group and the satisfactory group in preoperative SF-36 score. At 6 months after operation, the SF-36 scores of the two groups were significantly different in all 8 aspects, but there was no significant difference in the JOA scores between the two groups. Conclusion: there is a positive correlation between lower preoperative QOL score and postoperative satisfaction in patients with physical pain, general health and vitality after posterior lumbar interbody fusion.
