体外冲击波对早中期膝关节骨性关节炎患者关节液中IL-1β、TNF-α及MMP-13表达的影响

【摘要】：[目的]观察体外冲击波对膝关节骨性关节炎患者关节液中IL-1β、TNF-α及MMP-13表达的影响,探讨其治疗膝关节骨性关节炎的机制。[方法]将63例膝关节骨性关节炎患者分为两组,治疗组采用体外冲击波治疗,对照组采用塞来昔布胶囊治疗,均治疗4周,分别在治疗前后及治疗后6个月检测两组患者关节液中IL-1β、TNF-α及MMP-13的含量。[结果]治疗后,两组患者关节液中IL-1β、TNF-α及MMP-13的含量均明显低于治疗前(P0.05),其中治疗组与对照组含量相当(P0.05)。治疗后6个月随访,两组患者关节液中IL-1β、TNF-α及MMP-13的含量均较治疗后上升(P0.05),但仍明显低于治疗前(P0.05),而治疗组的含量明显低于对照组(P0.05)。[结论]体外冲击波能有效地下调膝关节骨性关节炎患者关节液中IL-1β、TNF-α及MMP-13的表达,从而抑制炎症反应,延缓病程发展,且与塞来昔布相比疗效更佳。
[Abstract]:[objective] to observe the effect of extracorporeal shock wave on the expression of IL-1 尾, TNF- 伪 and MMP-13 in the articular fluid of patients with knee osteoarthritis, and to explore the mechanism of the treatment of knee osteoarthritis. [methods] Sixty-three patients with knee osteoarthritis were divided into two groups. The treatment group was treated with extracorporeal shock wave and the control group with celecoxib capsule for 4 weeks. The contents of IL-1 尾, TNF- 伪 and MMP-13 in articular fluid were measured before and after treatment. [results] after treatment, the contents of IL-1 尾, TNF- 伪 and MMP-13 in articular fluid of the two groups were significantly lower than those before treatment (P0.05), and the contents of IL-1 尾, TNF- 伪 and MMP-13 in the treated group were similar to those in the control group (P0.05). Six months after treatment, the levels of IL-1 尾, TNF- 伪 and MMP-13 in articular fluid of the two groups were higher than those after treatment (P0.05), but still significantly lower than those before treatment (P0.05). The content of the treatment group was significantly lower than that of the control group (P0.05). [conclusion] Extracorporeal shock wave can effectively down-regulate the expression of IL-1 尾, TNF- 伪 and MMP-13 in the articular fluid of patients with knee osteoarthritis, thus inhibit inflammatory reaction, delay the course of disease, and the curative effect is better than celecoxib.
