肺挫伤早期血清TNF-α、IL-1β、IL-10、IL-13与肺超微结构以及肺功能的变化

【摘要】:目的通过建立肺挫伤模型,研究肺挫伤早期血清炎症因子TNF-α、IL-1β、IL-10、IL-13与肺超微结构以及肺功能的变化方法将16只新西兰白兔随机分为:(1)严重胸外伤肺挫伤组(n=12);(2)正常无损伤对照组(n=4)。建立严重胸外伤肺挫伤模型,于挫伤后1、2、3、4h分别检测血清IL-10、IL-13、IL-1β及TNF-α含量,同时在上述各个时相获取呼吸频率f、潮气量(VT)、气道压力、呼气末CO2浓度及呼吸流率曲线的数据,在各时相解剖肺组织,观察其超微结构病理的改变结果肺挫伤后早期TNF-α呈高表达状态,IL-1β表达升高,IL-10、IL-13表达总体上持续下降,肺超微结构进行性破坏,最终肺泡壁损害,肺泡腔内及间隔间大量红细胞淤积及炎性细胞渗出,毛细血管充血严重,导致肺泡通气量及PO2明显下降,P-AO2持续加大,肺顺应性下降。结论肺挫伤早期血清IL-1β、TNF-α表达明显升高,IL-10、IL-13表达降低;肺挫伤早期肺组织超微结构进行性破坏,肺功能持续降低;肺挫伤早期血清炎症因子的变化与肺超微结构及肺功能的改变可能有一定相关性
[Abstract]:Objective to study the serum inflammatory factors TNF- 伪, IL-1 尾 and IL-10, in early stage of lung contusion by establishing pulmonary contusion model. Methods 16 New Zealand white rabbits were randomly divided into two groups: (1) Pulmonary contusion group (n = 12); (2) normal control group (n = 4). The lung contusion model of severe thoracic trauma was established. The levels of serum IL-10,IL-13,IL-1 尾 and TNF- 伪 were measured at 1h, 2h, 3h after contusion, and the respiratory frequency f and tidal volume (VT), airway pressure were obtained at the same time. The data of end-expiratory CO2 concentration and respiratory flow rate curve were used to anatomize the lung tissue at each phase. The ultrastructural and pathological changes of the lung tissue were observed. The results showed that the expression of TNF- 伪 was high, the expression of IL-1 尾 was increased, and the expression of IL-10, was increased in the early stage of pulmonary contusion. The expression of IL-13 decreased continuously, the ultrastructure of lung was destroyed gradually, the alveolar wall was damaged, a large number of red blood cells and inflammatory cells exudated in and between alveolar cavity and septum, and the capillary congestion was serious. As a result, alveolar ventilation volume and PO2 decreased significantly, P-AO2 continued to increase and lung compliance decreased. Conclusion the expression of IL-1 尾 and TNF- 伪 in serum increased significantly and the expression of IL-10,IL-13 decreased in the early stage of pulmonary contusion, and the ultrastructure of lung tissue was gradually destroyed and the lung function was continuously decreased in the early stage of pulmonary contusion. The changes of serum inflammatory factors may be related to the changes of pulmonary ultrastructure and pulmonary function in the early stage of pulmonary contusion.
