双钢板内固定治疗肱骨远端粉碎性骨折临床分析

【摘要】：目的探讨双钢板内固定治疗肱骨远端粉碎性骨折临床疗效。方法回顾性分析自2010年5月-2014年10月在我院采用双钢板固定治疗肱骨远端粉碎性骨折患者33例,3例失访,有效例数为30例,30例成功收集全部患者临床资料,包括手术资料与术后疗效。其中男性18例,女性12例,年龄范围20-60岁,平均年龄36岁。按照AO/ASIF分型：C1型：12例,C2型10例,C3型8例,4例术前有尺神经损害,1例术前有桡神经损伤。均有外伤史,其中摔伤7例,高处坠落伤13例,车祸伤10例。受伤到手术时间3-8d,平均5d。通过对患者的肘、腕关节的疼痛、运动、X线表现以及肘关节功能评分进行研究。结果：随访时间6-24个月,平均18个月。根据Morrey肘关节功能评价标准对术后患者的肘关节功能进行评,疗效优14例(95-100分),良好11例(80-94分),中4例(50-79分),差1例(50分),总优良率为83.4%。结论：本组30例均为肱骨远端粉碎性骨折,通过双钢板内固定治疗,术后随访临床可获得较满意的肘关节活动度,无骨折不愈合,异位骨化1列,尺神经损伤1例,双钢板内固定治疗肱骨远端粉碎性骨折是一种较好的临床方法,具有生物力学的优势,固定牢靠疗效确切,并发症少,临床疗效满意。
[Abstract]:Objective to investigate the clinical effect of double plate fixation for comminuted distal humeral fractures. Methods from May 2010 to October 2014, 33 cases of comminuted fractures of distal humerus were treated with double plate fixation in our hospital. Including surgical data and postoperative results. There were 18 males and 12 females, aged 20-60 years, with an average age of 36 years. According to AO/ASIF classification: C1 type: 12 cases, C2 type 10 cases, C3 type 8 cases, 4 cases had ulnar nerve damage before operation, 1 case had radial nerve injury before operation. There were 7 cases of fall injury, 13 cases of falling injury and 10 cases of traffic accident injury. The time from injury to surgery was 3-8 days, with an average of 5 days. The pain, movement, X-ray findings and functional score of the elbow and wrist were studied. Results: the follow-up time was 6-24 months (mean 18 months). According to Morrey's elbow function evaluation standard, 14 cases (95-100 points) had excellent results, 11 cases (80-94 points) were good, 4 cases (50-79 points), 1 case (50%) poor, 4 cases (50-79 points), 1 case (50%) poor. The total excellent and good rate was 83.4%. Conclusion: all 30 cases of comminuted fractures of distal humerus were treated with double plate internal fixation. Satisfactory elbow motion, no fracture nonunion, ectopic ossification 1 column, ulnar nerve injury 1 case were obtained after operation. The treatment of comminuted fracture of distal humerus with double plate internal fixation is a good clinical method with the advantages of biomechanics, firm fixation, few complications and satisfactory clinical effect.
