温度对椎体后凸成形术中骨水泥灌注压力的影响

【摘要】:第一部分椎体强化术骨水泥灌注压力测定装置的研制目的:设计并研制一套能够直接测定椎体强化术时骨水泥灌注压力的装置,为临床手术提供直观的压力数据参考,以减少骨水泥渗漏,降低手术风险。方法:根据椎体强化术的相关特点和椎体的解剖学结构,在已有的手术器械基础上,运用相关物理学原理,设计能够直接测定骨水泥灌注压力的装置。结果:设计并研制了能够直接测定椎体强化术时骨水泥灌注压力的测定装置。包括应变片、推杆、电桥电路、信号放大调理器和数据采集示波器。结论:此套装置可用于监控和测定椎体强化术中骨水泥的灌注压力,降低骨水泥渗漏,降低手术风险,减少并发症的发生。第二部分椎体后凸成形术中骨水泥灌注压力的测量目的:利用自主研制的椎体强化术骨水泥灌注压力测定装置研究室温20℃下和模拟体内温度37℃下椎体后凸成形术中骨水泥灌注压力在椎体内的变化。方法:取脊柱胸腰椎标本(T10-L4),共10个椎体,将椎体随机分为2组:第一组(室温20℃)、第二组(模拟体内温度37℃),分别对第一组和第二组利用上海凯利泰公司的椎体后凸成形术手术器械进行单侧椎体后凸成形术,采用自主研制的骨水泥灌注压力测定装置记录骨水泥的动态灌注压力。结果:椎体后凸成形术中骨水泥灌注压力随骨水泥灌注量的增加而升高。20℃组第一管骨水泥平均灌注压力为6.34±2.05N,最大灌注压力为10.74±3.32N;第二管骨水泥平均灌注压力为9.42±1.69N,最大灌注压力为18.18±1.77N。37℃组第一管骨水泥平均灌注压力为9.68±1.36N,最大灌注压力为17.95±1.72N;第二管骨水泥平均灌注压力为15.66±2.10N,最大灌注压力为23.06±2.10N。结论:1.椎体后凸成形术中骨水泥灌注压力受到温度及骨水泥灌注量影响。2.临床上行椎体后凸成形术时应考虑体内外温度梯度对骨水泥灌注压力的影响,应尽量缩短骨水泥灌注时间。3.椎体后凸成形术时实时监控骨水泥灌注压力可有效预防骨水泥渗漏,提高手术安全性。
[Abstract]:Objective: to design and develop a set of apparatus which can directly measure the pressure of bone cement perfusion during vertebral body augmentation, and to provide an intuitionistic data reference for clinical operation. To reduce bone cement leakage, reduce the risk of surgery. Methods: according to the related characteristics and anatomical structure of the vertebral body, based on the existing surgical instruments and relevant physics principles, a device was designed to directly measure the pressure of bone cement perfusion. Results: an apparatus was designed and developed to directly measure the pressure of bone cement perfusion during vertebra augmentation. Including strain gauge, push rod, bridge circuit, signal amplifier regulator and data acquisition oscilloscope. Conclusion: this device can be used to monitor and measure the pressure of bone cement perfusion, reduce the leakage of bone cement, reduce the risk of operation and reduce the incidence of complications.
