血府逐瘀胶囊对膝下动脉PTA术后疗效的影响

【摘要】：研究目的:本研究旨在探讨血府逐瘀胶囊对膝下动脉PTA术后疗效的影响。研究对象:选取2015年02月至2016年05月在中国中医科学院西苑医院周围血管病科住院,符合下肢动脉硬化闭塞症诊断,中医辨证存在血瘀证,而且在我科成功接受了膝下动脉PTA治疗的40例患者。研究方法:将入组的40例患者随机分入西药常规治疗组(对照组)和西药常规治疗+中药治疗组(治疗组),各20例。对照组口服阿司匹林100mg,qd;治疗组口服阿司匹林100 mg,qd+血府逐瘀胶囊6粒,bid(从术后第3天开始至术后半年随访终点停用)。在术前、术后3天及术后半年对患者进行相关实验室检查指标的检测、ABI测量、Rutherford分级的记录。随访结束,对两组患者这些指标数据进行统计学分析,评价血府逐瘀胶囊对膝下动脉PTA后疗效的影响。研究结果:1.两组患者在年龄、性别、病史、PTA治疗等方面情况基本相似,差异无统计学意义(P0.05),具有可比性。2.两组患者的空腹血糖(FBG)、糖化血红蛋白(HbAlc)、甘油三酯(TG)、低密度脂蛋白(LDL)、白介素-6(IL-6)、超敏C反应蛋白(hs-CRP)、纤维蛋白原(Fg)实验室检查指标在中药治疗前,统计学上无显著差异(P0.05);术后半年,治疗组较对照组各项指标均有明显改善,组间相比差异具有统计学意义(P0.05)。3.膝下PTA术后3天,评估治疗组与对照组之间Rutherford分级构成无显著性差异(P0.05)。术后半年,治疗组与对照组患者Rutherford分级组间存在差异,有统计学意义(P0.05)。4.膝下动脉PTA后第3天,测量两组患者ABI较术前均明显提高,但两组间无显著差异(P0.05);术后半年,治疗组ABI数值与对照组ABI数值有存在差异,有统计学意义(P0.05)。随访期间无不良事件发生。结论:血府逐瘀胶囊通过其抗动脉粥样硬化、扩张血管、改善血液流变性和微循环、降血脂等作用对膝下动脉PTA术后疗效的维持和改善有一定的干预作用,对患者的临床症状、体征及生活质量提高有积极的意义。
[Abstract]:Objective: to investigate the effect of Xuefu Zhuyu capsule on PTA of inferior genu artery. Participants: selected from February 2015 to May 2016 in the Department of Peripheral Angiology, Xiyuan Hospital, Chinese Academy of traditional Chinese Medicine, in accordance with the diagnosis of lower extremity arteriosclerosis obliteration, TCM syndrome differentiation exists blood stasis, And 40 patients who were successfully treated with PTA of inferior genu artery in our department. Methods: 40 patients were randomly divided into two groups: the control group (n = 20) and the traditional Chinese medicine group (n = 20). The control group took aspirin 100mg / g QD, and the treatment group took aspirin 100 mg,qd Xuefu Zhuyu capsule (6, bid () from the 3rd day after operation to the end of half a year after operation. Fasting blood glucose (FBG), glycosylated hemoglobin (HbAlc), triglyceride (TG), low density lipoprotein (LDL), interleukin-6 (IL-6), hypersensitive C-reactive protein (hs-CRP), There was no significant difference in fibrinogen (Fg) laboratory index before treatment with Chinese medicine (P0.05). Half a year after operation, the treatment group compared with the control group were significantly improved, the difference between the two groups was statistically significant (P0.05). There was no significant difference in the composition of Rutherford grade between the treatment group and the control group 3 days after PTA (P0.05). Half a year after operation, the treatment group and the control group between the Rutherford grading group differences, there was statistical significance (P0.05). On the 3rd day after PTA of inferior genu artery, the ABI of the two groups was significantly higher than that of the pre-operation, but there was no significant difference between the two groups (P0.05). Half a year after the operation, the ABI value of the treatment group and the control group were significantly different (P0.05). No adverse events occurred during follow-up. Conclusion: Xuefu Zhuyu capsule has some interventional effects on the maintenance and improvement of the curative effect of inferior genu artery PTA through its effects of anti-atherosclerosis, dilating blood vessels, improving blood rheology and microcirculation and lowering blood lipid. The improvement of physical signs and quality of life is of positive significance.
