成年人躯干核心肌耐力研究

【摘要】:[目的]建立成年男性和女性在不同年龄阶段躯干核心肌耐力测试的参考值范围。[方法]188位(男性:88位)年龄在20-59(35.9±13.3岁)之间的受试者自愿参与到本研究中,其中健康受试者172位,腰痛患者16位。实验开始前所有受试者均需填写一份基本信息表和签署知情同意书,腰痛患者还需根据当天的腰痛情况额外填写一份功能障碍指数量表(Oswestry Disability Index:ODI)和使用疼痛视觉模拟评分(Visual Analogue Scale:VAS)为自己当天的腰痛程度评分。所有受试者均需按要求完成索伦森测试、平板支撑、左侧搭桥和右侧搭桥四项测试,每项测试的顺序由抓阄随机决定,记录每项测试的时间,测试之间均有20分钟休息时间。将所有健康受试者按年龄段依次分为4个组,即20-29岁组,30-39岁组,40-49岁组,50-59岁组,计算不同年龄组的健康受试者和腰痛组每种测试的平均支撑时间。使用相关性分析计算ODI得分与索伦森测试、平板支撑、左侧搭桥实验、右侧搭桥实验的支撑时间以及VAS之间的相关系数。通过受试者特征曲线(Receiver Operator Characteristic curve,ROC 曲线),计算使用四种测试的总时间来区分腰痛患者和非腰痛患者的分界点值以及该点的灵敏度和特异度。[结果]健康受试者20-29岁组,30-39岁组,40-49岁组,50-59岁组索伦森测试的平均时间分别为:150±45s,117±38s,113±32s,125±36s。平板支撑的时间分别为:111±59s,84±39s,86±35s,91±51s。左侧和右侧搭桥实验的平均时间均在60-80s之间。除了 50-59岁年龄组的索伦森测试和平板支撑比30-39岁,40-49岁两个年龄组略高之外,其他测试均遵循随着年龄的增长,测试支撑的时间也随之减少的趋势。腰痛组,除了 VAS与ODI之间有高度相关性(rho = 0.813),剩下的每种测试的支撑时间与ODI均呈低相关性或者不相关(rho0.4)。ROC 曲线下面积(Area Under the Curve:AUC)为 0.723(0.573,0.865),最佳区分腰痛患者和非腰痛患者的时间分界点为288s,该分界点的灵敏度和特异度均为0.75。[结论]本实验建立了中国健康成年人的核心肌耐力参考值范围,但可能由于中国中老年人的生活习惯或者体重指数(Body Mass Index:BMI)与国外同龄阶段人群有所不同,导致50-59岁人群核心肌耐力测试时间与国外有所差异。此外,由于本实验样本量有限,所以关于50-59岁年龄的段核心肌耐力参考值范围还需今后做更多的研究证实。索伦森测试、平板支撑、左侧和右侧搭桥四种核心肌耐力测试的总时间区分腰痛患者和非腰痛患者的分界点为288s。
[Abstract]:Objective: to establish the reference range of trunk core muscle endurance test for adult male and female at different ages. [methods] 188 subjects (male: 88) aged between 20 and 59 (35.9 卤13.3) volunteered to participate in the study, including 172 healthy subjects and 16 patients with low back pain. Before the experiment began, all subjects were required to fill out a basic information form and sign an informed consent form. Patients with low back pain also need to fill in an additional low back pain index scale (Oswestry Disability Index:ODI) and use pain visual analogue scale (Visual Analogue Scale:VAS) to score the degree of low back pain on that day. All subjects were required to complete four tests: Sorenson test, plate support, left side bypass and right side bypass. The order of each test was randomly determined by drawing lots. The time of each test was recorded and there were 20 minutes of rest time between the tests. All healthy subjects were divided into 4 groups according to their age groups: 20-29, 30-39, 40-49, 50-59 years old. The average support time of each test was calculated for healthy subjects and low back pain groups. Correlation analysis was used to calculate the correlation coefficient between ODI score and Sorenson test, plate support, left side bypass test, right side bypass test and VAS. By means of the (Receiver Operator Characteristic curve,ROC curve of the subjects' characteristic curve, the total time of the four kinds of tests was calculated to distinguish the demarcation points between the patients with low back pain and those with non-low back pain, as well as the sensitivity and specificity of the points. [results] the average time of Sorenson test in healthy subjects aged 20-29, 30-39, 40-49, 50-59 years old was 150 卤45s, 117 卤38s, 113 卤32sn, 125 卤36s, respectively. The supporting time of the plate was 111 卤59 s-1, 84 卤39 sm, 86 卤35 s, 91 卤51 s, respectively. The average time of the left and right side bypass experiment was between 60 and 80 s. With the exception of the Sorenson test and the panel support ratio of 30-39 and 40-49 in the 50-59 age group, the other tests followed the trend that the duration of the test support decreased with the increase of age. In the low back pain group, except for the high correlation between VAS and ODI (rho = 0.813), For each of the remaining tests, there was a low or no correlation between the support time and the ODI (area under the rho0.4). ROC curve, (Area Under the Curve:AUC) was 0.723 (0.573 卤0.865), The best time boundary point for distinguishing low back pain patients from non low back pain patients is 288 s, and the sensitivity and specificity of this boundary point are both 0. 75. [conclusion] the reference range of core muscle endurance of Chinese healthy adults was established in this experiment, but it may be due to the difference of life habits or body mass index (Body Mass Index:BMI) between Chinese and foreign age groups. As a result, the test time of core muscle endurance of 50-59 years old population was different from that of foreign countries. In addition, due to the limited sample size of the experiment, the reference range of the core muscle endurance between 50 and 59 years old needs to be confirmed by more studies in the future. The total time of Sorenson test, plate support, left side and right side bypass test to distinguish low back pain patients from non low back pain patients was 288 s.
