大脑半球多脑叶离断术治疗难治性癫痫的临床研究

【摘要】:目的 探讨大脑半球离断术治疗半球性病变导致的难治性癫痫的手术方法及疗效。方法 回顾性收集宁夏医科大学总医院神经外科2014年8月至2016年1月全部采用大脑半球离断手术治疗难治性癫痫患者的临床资料,包括病史,临床症状,影像学资料,视频脑电图检测结果,采用相同的手术方术,对患侧大脑半球进行分步离断:先经侧裂切除海马杏仁核;经颞角沿三角区向额角分段皮层造瘘,分段离断额叶、顶叶、枕叶与胼胝体的联系;最后经侧裂切除岛叶皮质。结果 该术式在4例患者成功实施。术后4例患者获全程随访,随访时间8-20个月,平均13个月。结果证实,手术创伤小,并发症少,术后病人没有任何加重对侧肢体功能障碍的并发症,癫痫得到有效控制,Engel I级3例,EngelⅡ级1例。且术后恢复好,神经心理,精神行为明显改善。结论 大脑半球多脑叶离断术是一种对半球性病变导致的难治性癫痫患者创伤小、疗效可靠的治疗方案。
[Abstract]:Objective To study the surgical method and curative effect of cerebral hemisphere off-cut in the treatment of intractable epilepsy caused by hemispheric lesion. Methods The clinical data from the neurosurgical department of the general hospital of Ningxia Medical University from August 2014 to January 2016 were retrospectively collected, including medical history, clinical symptoms, imaging data and video and EEG detection results. In the same procedure, the cerebral hemisphere of the affected side was divided by step: firstly, the amygdala of the hippocampus was cut off by the lateral fissure; the contact of the frontal lobe, the parietal lobe, the occipital lobe and the hippocampus of the frontal lobe of the frontal angle was divided into the frontal angle of the frontal angle along the triangle, and the cortex of the island of the island was cut through the lateral fissure. Results The operation was performed in 4 patients. The follow-up time was 8-20 months and the mean 13-month follow-up.
