术中输注不同保存时间库存红细胞对老年髋关节置换术患者术后谵妄的影响

【摘要】：目的:探讨术中输注不同保存时间库存红细胞对老年髋关节置换术患者术后谵妄的影响。方法:选取老年髋关节置换术患者110例,采用随机数字表法分为两组,对照组患者55例术中输注保存时间14 d的库存红细胞,观察组患者55例术中输注保存时间≤14 d的库存红细胞,比较两组患者手术情况、术后谵妄的发生情况。结果:观察组患者输血4 h、1 d、3 d时血红蛋白高于对照组(P0.05),C反应蛋白、白介素-6、肿瘤坏死因子α水平均低于对照组(P0.05),两组患者手术时间、住院时间、晶体液量、胶体液量、尿量、术中出血量比较,差异均无统计学意义(P0.05),观察组患者术后并发症发生率、术后谵妄发生率低于对照组(P0.05),观察组患者术后谵妄持续时间少于对照组(P0.05)。结论:术中输注保存时间≤14 d库存红细胞可有效降低老年髋关节置换术患者术后谵妄发生的效果优于术中输注保存时间14 d的库存红细胞的效果。
[Abstract]:Objective: to investigate the effect of intraoperative infusion of red blood cells at different storage times on postoperative delirium in elderly patients undergoing hip arthroplasty. Methods: 110 elderly patients undergoing hip arthroplasty were randomly divided into two groups: control group (n = 55) and control group (n = 55). 55 patients in the observation group received intraoperative infusion of red blood cells less than 14 days. The operative and postoperative delirium were compared between the two groups. Results: the levels of), C reactive protein, interleukin-6 and tumor necrosis factor-伪 in the observation group were lower than those in the control group at 4 hours, 1 day and 3 days after blood transfusion (P0.05), and the operation time of the two groups was significantly lower than that of the control group (P0.05). There was no significant difference in hospitalization time, volume of lens fluid, colloid fluid, urine volume and intraoperative bleeding (P0.05). The incidence of postoperative complications and postoperative delirium in the observation group was lower than that in the control group (P0.05).
