非剥脱性点阵激光治疗增生性瘢痕的临床研究

【摘要】:背景和目的:在皮肤组织受到创伤后,由于受到各方面的影响在愈合过程中会出现胶原纤维的过度增生,形成了增生性瘢痕或者瘢痕疙瘩。各种形式的方法已经被用于治疗增生性瘢痕,但由于有限的有效性和不可忽视的副作用从而限制了它们的合理应用。最近,一种新型的非剥脱性的点阵激光技术也开始被应用于增生性瘢痕的治疗中。本文的目的是评估非剥脱性点阵激光治疗17例增生性瘢痕患者的有效性及安全性。方法:我科门诊就诊的增生性瘢痕患者17例被纳入研究,给予非剥脱性点阵激光治疗。告知患者此次治疗共6次,每隔4-6周来院治疗一次。治疗结束后随访半年。每次治疗前及治疗结束后一月由两名医师根据数码相机拍摄照片及患者瘢痕表象对治疗效果进行评价,治疗前和6次治疗后由两名医师进行VSS评分。疗程结束后一月由患者本人对治疗效果的满意程度进行评分。结果:17例患者都完成6次治疗,次数增加疗效也随之提高,6次治疗后的总有效率为58.8%。根据温哥华瘢痕评分量表(VSS)6次治疗后的总有效率为64.7%。患者满意度自为58.8%。治疗后患者感轻微疼痛,治疗部位轻度水肿,局部给予冰袋冰敷30分钟症状缓解。治疗结束后没有患者治疗部位出现水疱、结痂等。随访半年后,1例患者出现治疗部位色素沉着,未经过任何方法进行干预,半年后逐渐好转。结论:非剥脱性点阵激光是一种有效的、安全的治疗增生性瘢痕的方法,值得我们进一步研究及应用临床。
[Abstract]:Background and objective: hypertrophic scar or keloid will occur in the process of healing due to the excessive proliferation of collagen fibers in the process of healing after trauma of skin tissue. Various forms of methods have been used in the treatment of hypertrophic scars, but their reasonable application is limited by their limited effectiveness and side effects that can not be ignored. Recently, a new type of non-exfoliative lattice laser technique has also been used in the treatment of hypertrophic scars. The purpose of this study was to evaluate the efficacy and safety of non-stripping lattice laser in the treatment of 17 patients with hypertrophic scar. Methods: seventeen patients with hypertrophic scar in our clinic were treated with non-stripping lattice laser. Inform the patient of this treatment 6 times, every 4 times 6 weeks to come to the hospital for treatment. The patients were followed up for half a year after treatment. Before and one month after each treatment, the therapeutic effect was evaluated by two doctors according to the photos taken by digital camera and the scar appearance of the patients, and the VSS scores were evaluated by two doctors before and 6 times after treatment. One month after the end of the course of treatment, the patients' satisfaction with the therapeutic effect was evaluated. Results: all the 17 patients completed 6 times of treatment, and the curative effect was also improved with the increase of frequency. The total effective rate was 58.8% after 6 times of treatment. According to the Vancouver scar scale (VSS), the total effective rate was 64.7%. The patient satisfaction rate was 58.8%. After treatment, the patients felt mild pain, mild edema in the treatment site, and local ice bag ice compress for 30 minutes to relieve the symptoms. There were no blisters and scabs in the treatment site after treatment. After half a year of follow-up, one patient had pigmentation at the treatment site, and gradually improved after half a year without any intervention. Conclusion: non-stripping lattice laser is an effective and safe method for the treatment of hypertrophic scar, which is worthy of further study and clinical application.
