空心钉内固定治疗股骨颈骨折:关节功能恢复的影响因素分析

【摘要】：背景:目前,髋关节置换和空心加压螺钉内固定是治疗老年股骨颈骨折的主要方法。但老年患者由于骨质疏松,内固定往往失败。目的:分析空心钉内固定治疗股骨颈骨折患者术后关节功能,并探索影响空心钉内固定后髋关节功能的相关因素。方法:回顾性分析空心钉内固定治疗的股骨颈骨折患者并获得完整2年随访的89例患者临床资料,就性别、年龄、骨折类型(Garden分型)、骨量、体质量指数、空心钉固定结构、复位指数、手术时间、内固定后至患肢部分负重时间及2年随访时髋关节功能Harris评分等因素进行分析。应用SPSS 18.0统计学软件进行等级相关分析、有序多分类logistic回归分析。结果与结论:(1)标准正态Q-Q概率图及正态性检验结果显示,所有获得随访的患者术后2年Harris评分呈偏态分布(P=0.000);(2)等级秩相关分析显示:年龄、骨折类型、骨量、复位指数与髋关节功能评分呈相关性(P0.05或P0.01);(3)有序多分类Logistic回归分析结果显示:年龄、骨折类型、骨量、复位指数4个自变量对髋关节功能评分的影响具有显著性意义;(4)结果说明,年龄、骨折类型、骨量、复位指数是空心钉内固定治疗股骨颈骨折术后髋关节功能的重要影响因素。
[Abstract]:Background: hip replacement and hollow compression screw fixation are the main methods for the treatment of femoral neck fracture in the elderly. However, due to osteoporosis, internal fixation often fails in elderly patients. Objective: to analyze the joint function of patients with femoral neck fracture treated with hollow nail internal fixation, and to explore the related factors affecting hip joint function after hollow nail internal fixation.
