ConView脑电意识状态法与BIS法监测患者异丙酚镇静深度准确性的比较

【摘要】:目的:通过与BIS法比较,评价ConView脑电意识状态法监测患者异丙酚镇静深度的准确性。方法:全麻下行择期手术患者30例,性别不限,年龄41~64岁,体重指数18~28kg/m2,ASA分级Ⅰ或Ⅱ级。常规术前准备。入室后建立肘正中静脉通路,输注复方氯化钠注射液,常规监测ECG、HR、SpO2和MAP。按照要求放置BIS与ConView脑电意识状态监测仪专用传感器电极片,确保各电极皮肤电阻小于5.0kΩ,信号质量指数(Signal Quality Index,SQI)大于90。采用Marsh药代动力学参数TCI输注异丙酚,设定初始Ce为1.2μg/ml,待Ce稳定后以0.3μg/ml为梯度递增,并同时行OAA/S评分。记录开始输注异丙酚前进行OAA/S评分及TCI异丙酚期间OAA/S评分依次为4分、3分、2分、1分时的BIS值、AI值和异丙酚Ce。维持生命体征指标在正常范围内。对BIS值、AI值及Ce分别与OAA/S评分行Spearman等级相关分析,对BIS值、AI值分别与Ce行Pearson相关分析,相关系数差异的显著性检验采用u检验,以P0.05为差异有统计学意义。结果:BIS值、AI值、Ce与OAA/S评分的相关系数分别为rBIS-OAA/S=0.962、rAI-OAA/S=0.960、rCe-OAA/S=㧟0.795(P0.05);BIS值、AI值与Ce的相关系数分别为rBIS-Ce=㧟0.736、rAI-Ce=㧟0.737(P0.05)。rBIS-OAA/S及rAI-OAA/S之间、rBIS-Ce及rAI-Ce之间比较差异均无统计学意义(P0.05)。结论:成年患者非伤害性刺激条件下,ConView脑电意识状态法监测患者异丙酚镇静深度准确,且与BIS法相比无明显差异。
