肝胆管结石相关肝内胆管癌的临床资料分析

　　目的1.探讨肝胆管结石相关肝内胆管癌（HL-ICC）的危险因素及临床特征,提高诊断和治疗水平。2.研究肿瘤标志物CA199、癌胚抗原CEA对肝胆管结石相关肝内胆管癌的诊断价值。3.比较增强CT、增强MRI、B超在肝胆管结石相关肝内胆管癌中的诊断效果。方法:采用回顾性病例对照研究,收集湖南省人民医院胆道外一科2010年1月—2012年12月间,其中肝胆管结石相关肝内胆管癌患者有63例被纳入为肿瘤组和同期单纯肝胆管结石患者192例纳入结石组,对相关因素采用单因素,并对单因素分析中存在统计学差异的因素采取logistic回归模型多因素分析。收集上述病例一般资料、血清学指标、影像学资料以及病理活检结果,结合两组病人收集的临床数据分析影响诊断的因素。结果:1.单因素分析显示,存在统计学差异的因素有:发病早期取石（P=0.002）、家族肿瘤史（P<0.001）、胆肠吻合手术史（P=0.022）、乙肝病史（P=0.023）。对单因素分析中存在统计学差异的因素采用Logistic回归分析结果显示:胆肠吻合手术史（OR=5.114,95%CI:1.734-15.097,P=0.004）,家族肿瘤史...
