【摘要】:目的评估术前是否合并有睑板腺功能障碍(Meibomian gland dysfunction, MGD)以及MGD严重程度对年龄相关性白内障超声乳化术后视觉质量的影响。方法前瞻性临床研究。术前根据患者是否合并有MGD以及MGD的严重程度分为A、B、C三组:A组28例(35眼)术前无MGD,B组26例(35眼)术前合并有轻度MGD,C组28例(35眼)术前合并有中度MGD。术后随访1天、1周、1月、≥3月(3.44±0.44月),检查并记录裸眼远视力(uncorreted distance visual acuity, UDVA)、3mm瞳孔直径下的总高阶像差(HO Total)、慧差(Coma)、球差(Spherical)、三叶草(Trefoil)、视觉质量满意度问卷调查以及泪膜破裂时间(tear break up time,BUT)。结果1)术后1天,A组和B组、A组和C组、B组和C组UDVA、HO Total、 Coma、Spherical、TrefoIL、视觉质量满意度比较差异无统计学意义。2)术后1周,A组和B组、A组和C组HO T otal、Coma、Trefoil匕较差异有统计学差异(P0.05), UDVA、Spherical、视觉质量满意度比较差异无统计学意义;B组和C组UDVA、HO Total、Coma、Spherical、Trefoil、视觉质量满意度比较差异无统计学意义。3)术后1月,A组和B组、A组和C组Coma比较差异有统计学差异(P0.05,UDVA、HO Total、Spherical、Trefoil、视觉质量满意度比较差异无统计学意义;B组和C组UDVA、HO Total、Coma、Spherical、Trefoilv视觉质量满意度比较差异无统计学意义。4)术后≥3月,A组和B组、A组和C组、B组和C组HO Total、Trefoil比较差异有统计学差异(P0.05); UDVA、Coma、Spherical比较差异无统计学意义。5)MGD患者(B组和C组)术后视觉质量与BUT的相关性分析得出,术后≥3月HO Total、Trefoil与BUT的相关性有统计学意义(P0.05),且呈负相关;Coma、Spherical与BUT的相关性无统计学意义。术后1天、1周、1月H0 Total、Coma、Spherical、Trefoil与BUT的相关性无统计学意义。6)术后视觉质量与MGD等级的回归分析得出,术后≥3月HO Total、Trefoil与MGD等级的回归分析有统计学意义(P0.05),Coma、Spherical与MGD等级的回归分析无统计学意义。术后1天、1周、1月H0 Total、Coma、Spherical、Trefoil与MGD等级的回归分析无统计学意义。结论1)术前无MGD患者术后视觉质量要优于术前合并有MGD患者。2)术后≥3月,MGD患者的视觉质量与泪膜稳定性呈正相关。3)术后≥3月,患者的视觉质量与MGD等级呈负相关。
[Abstract]:Objective to evaluate the effect of Meibomian gland dysfunction (MGD) and the severity of MGD on visual quality of age-related cataract after phacoemulsification. Methods a prospective clinical study. According to whether the patients were combined with MGD and the severity of MGD, they were divided into three groups: A, B and C. There were 28 cases (35 eyes) in A group, 26 cases (35 eyes) in B group, 26 cases (35 eyes) in B group, 28 cases in preoperative group, 28 cases in C group (35 eyes). The postoperative follow-up was 1 days, 1 week, January, March (aged 3.44 + 0.44 months), check and record the uncorrected visual acuity (uncorreted distance visual acuity, UDVA), the total higher-order aberrations of the pupil diameter of 3mm (HO Total), coma (Coma), spherical aberration (Spherical) and Clover (Trefoil), visual quality satisfaction questionnaire and tear break-up time (tear break up time, BUT). Results 1) 1 days after operation, there was no significant difference in UDVA, HO Total, Coma, Spherical, and visual quality of A group and B group, A group and C group, B group and C group. 2) 1 weeks after operation, A group and B group, A group and C group, HO T otal, Coma, Trefoil, there was significant difference between the difference (P0.05), UDVA, Spherical, visual quality satisfaction, no significant difference was not statistically significant; B group and C group UDVA, HO Total, Coma, Spherical Trefoil and visual quality satisfaction difference. 3) after January, there is significant difference between the A group and B group, A group and C group Coma (P0.05, UDVA, HO between Total, Spherical, Trefoil, visual quality satisfaction was no statistically significant difference between B group and C group; UDVA, HO Total, Coma, Spherical, Trefoilv visual quality satisfaction comparison there was no statistically significant difference. 4) after more than March, there is significant difference between the A group and B group, A group and C group, B group and C group, HO Total Trefoil (P0.05); the difference was not statistically significant UDVA, Coma, Spherical difference. 5) MGD patients (B group and C group) and BUT analysis of the correlation of visual quality after that, after more than Total, Trefoil and HO between March BUT was statistically significant (P0.05), and was negatively correlated with Coma, Spherical and BUT; the correlation was not statistically significant. There was no statistically significant correlation between H0 Total, Coma, Spherical, Trefoil and BUT in 1 days, 1 weeks and January after the operation. 6) regression grade postoperative visual quality and MGD analysis, after the March HO Total Trefoil, or regression analysis with MGD grade was statistically significant (P0.05), Coma, Spherical and MGD level regression analysis was not statistically significant. There was no statistical significance in the regression analysis of H0 Total, Coma, Spherical, Trefoil and MGD levels in 1 days, 1 weeks and January after the operation. Conclusion 1. The visual quality of patients without MGD before operation is better than that of patients with MGD before operation. 2) after more than March, the visual quality of patients with MGD was positively related with the stability of tear film. 3) after more than March, the visual quality of patients was negatively correlated with MGD level.
