鼓室内注射与耳后注射甲基泼尼松龙治疗难治性突发性耳聋效果比较

【摘要】：目的比较鼓室内注射与耳后注射甲基泼尼松龙治疗难治性突发性耳聋的效果。方法选取温州市中心医院2014年3月至2015年12月确诊并收治的62例难治性耳聋患者,按随机数字表法分为观察组和对照组,每组31例。所有患者根据病情持续使用微循环优化、营养神经等常规治疗方案。在此基础上,观察组采取鼓室内注射甲基泼尼松龙,对照组采取耳后注射甲基泼尼松龙,对比两组疗效、各频率听力改善情况及不良反应。结果观察组总有效率略高于对照组,但差异无统计学意义。两组各频率平均听阈均有所改善,在0.5、1及3k Hz听阈的改善幅度组间无明显差异,而观察组在4、8k Hz听阈的改善幅度明显优于对照组。两组不良反应均轻微、可耐受,自行缓解。结论鼓室内注射与耳后注射甲基泼尼松龙治疗难治性突发性耳聋效果均较好,而耳后注射方案主要针对低、中频听力下降的患者。
[Abstract]:Objective to compare the effect of intratympanic injection and retroauricular injection of methylprednisolone in the treatment of refractory sudden deafness. Methods from March 2014 to December 2015, 62 patients with refractory deafness in Wenzhou Central Hospital were selected and divided into observation group (n = 31) and control group (n = 31). All patients continued to use microcirculation optimization, nutritional nerve and other routine treatment according to the condition. On this basis, the observation group received intratympanic injection of methylprednisolone and the control group received retroauricular injection of methylprednisolone. Results the total effective rate of the observation group was slightly higher than that of the control group, but the difference was not statistically significant. The average hearing threshold of each frequency in the two groups was improved, and there was no significant difference between the two groups in the improvement amplitude of 0.5kHz and 3kHz hearing threshold, but the improvement range of the observation group was obviously better than that of the control group at 4m 8kHz. The adverse reactions in both groups were mild, tolerable and self-remission. Conclusion both intratympanic injection and retroauricular injection of methylprednisolone are effective in the treatment of refractory sudden hearing loss.
