阻塞性睡眠呼吸暂停低通气综合征相关性高血压患者血清胱抑素C和超氧化物歧化酶水平的变化

【摘要】:目的:观察阻塞性睡眠呼吸暂停低通气综合征(OSAS)相关性高血压患者血清胱抑素C(cystatine,CysC)和超氧化物歧化酶(Superoxide dismutase,SOD)水平的变化。方法:收集2013-01至2013-12在甘肃省人民医院心内科和鼾病科就诊并完成多导睡眠监测的高血压患者102例,根据呼吸暂停低通气指数(Apnea hypopnea index,AHI)将102例高血压患者分成2组:AHI≥5次/h的纳入OSAS相关性高血压组,即OH组55例,AHI5次/h的纳入单纯高血压组,即高血压组47例。另选取同时间段内完成多导睡眠监测的健康人45名作为对照组。对所有研究对象进行一般情况、生化指标、睡眠参数方面数据的整理,采用SPSS17.0软件分析并比较3组上述指标间差异是否有统计学意义。结果:与对照组比较,OH组、高血压组的血清CysC水平[(1.06±0.27)mg/L和(0.91±0.19)mg/L vs(0.79±0.32)mg/L]明显升高,血清SOD水平[(127.63±63.19)U/ml和(149.14±60.12)U/ml vs(181.54±59.37)U/ml]明显下降,组间比较差异均有统计学意义(P均0.05)。进一步将OH组按OSAS病情分为轻度者、中度者、重度者3个亚组,并将3个亚组结果进行比较可见,血清CysC、SOD在轻度和中度、轻度和重度以及中度和重度间比较,差异均无统计学意义(P均0.05)。结论:高血压组和OH组血清CysC水平高于对照组,而血清SOD水平低于对照组;且OH组血清CysC水平高于高血压组,血清SOD低于高血压组(P0.05),但其水平变化与OSAS病情程度的关系尚未证实。
[Abstract]:Aim: to observe the changes of serum cystatin (CysC) and superoxide dismutase (SOD) levels in patients with obstructive sleep apnea hypopnea syndrome (OSAS) associated hypertension. Methods: 102 patients with hypertension who were admitted to Department of Cardiology and snoring Department of Gansu Provincial people's Hospital from January to December 2013 and completed polysomnography were collected. According to apnea hypopnea index (AHI), 102 patients with hypertension were divided into two groups: OSAS group (n = 55) and hypertension group (n = 47). Another 45 healthy people who completed polysomnography in the same time period were selected as control group. The data of biochemical indexes and sleep parameters were analyzed and compared by SPSS17.0 software. Results: compared with the control group, the serum CysC levels [1.06 卤0.27)mg/L and 0.91 卤0.19)mg/L vs(0.79 卤0.32)mg/L] were significantly increased, and the serum SOD levels [127.63 卤63.19)U/ml and 149.14 卤60.12)U/ml vs(181.54 卤59.37)U/ml] were significantly decreased in the hypertensive group (P < 0.05). The OH group was further divided into 3 subgroups according to the condition of OSAS: mild group, moderate group and severe subgroup. The results of the three subgroups were compared. Serum CysCnsSOD was compared between mild and moderate, mild and severe, moderate and severe. The difference was not statistically significant (P = 0.05). Conclusion: the serum CysC level in hypertension group and OH group is higher than that in control group, while the serum SOD level in OH group is lower than that in control group, and the serum CysC level in OH group is higher than that in hypertension group. The level of serum SOD was lower than that of hypertension group (P 0.05), but the relationship between the level of serum SOD and the severity of OSAS was not confirmed.
