透明角膜和巩膜隧道切口白内障超声乳化联合人工晶体植入术后干眼症发生情况比较

【摘要】：目的探讨透明角膜、巩膜隧道两种不同切口白内障超声乳化联合人工晶体植入术后干眼症发病情况。方法选取2013年9月—2016年9月于南阳医专第一附属医院行白内障超声乳化联合人工晶体植入术200例白内障患者,根据手术方式不同分为观察组(采用巩膜隧道切口,100例)和对照组(采用透明角膜切口,100例)。比较两组手术前后干眼症评分、角膜荧光素染色评分、泪膜破裂时间、基础泪液分泌量。结果观察组术后1周、术后1个月干眼症、角膜荧光素染色评分低于对照组(P0.05)。两组术后1周、术后1个月干眼症、角膜荧光素染色评分均高于术前(P0.05)。观察组术后1周、术后1个月泪膜破裂时间、基础泪液分泌量高于对照组(P0.05)。两组术后1周、术后1个月泪膜破裂时间、基础泪液分泌量均低于术前(P0.05)。结论巩膜隧道切口在白内障超声乳化联合人工晶体植入术的应用中相较于透明角膜在术后1个月内对泪膜的稳定性影响较轻,显著降低干眼症症状。
[Abstract]:Objective to investigate the incidence of dry eye after phacoemulsification and intraocular lens implantation in two different incisions of transparent cornea and scleral tunnel. Methods from September 2013 to September 2016, 200 patients with cataract underwent phacoemulsification combined with intraocular lens implantation in the first affiliated Hospital of Nanyang Medical College. The patients were divided into observation group (100 cases with scleral tunnel incision) and control group (100 cases with transparent corneal incision). The dry eye score, corneal fluorescein staining score, tear film rupture time and basal tear secretion were compared between the two groups before and after operation. Results the corneal fluorescein staining score in the observation group was lower than that in the control group at 1 week and 1 month postoperatively. The corneal fluorescein staining score in both groups was higher than that in preoperation (P 0.05). The time of tear film rupture in the observation group was higher than that in the control group at 1 week and 1 month after operation, and the amount of basal tear secretion was higher than that in the control group (P 0.05). The time of tear film rupture and the amount of basal tear secretion in both groups were lower than those before operation (P 0.05) 1 week after operation and 1 month after operation. Conclusion the application of scleral tunnel incision in cataract phacoemulsification combined with intraocular lens implantation has less effect on the stability of tear film than that of transparent cornea within one month after operation.
