玻璃体切除术治疗增殖性糖尿病视网膜病变的临床研究

【摘要】：目的探讨玻璃体切除术对增殖性糖尿病视网膜病变(PDR)患者的临床治疗疗效。方法将112例PDR患者180只眼以数字表法随机分为观察组(58例,90眼)与对照组(54例,90眼),对照组给予光凝治疗,观察组给予玻璃体切除术治疗,比较两组患者的治疗效果。结果治疗后观察组Ⅳ期好转率为84.4%,Ⅴ期好转率为64.3%,Ⅵ期好转率为50.0%,均明显高于对照组的27.6%、23.3%以及16.1%,差异均有统计学意义(P0.05);观察组患者继发性青光眼发生率为1.1%,明显低于对照组的14.4%(P0.05),其他并发症发生率比较则差异无统计学意义(P0.05)。结论玻璃体切除术在PDR患者中具有确切的疗效,可有效改善患者视力,同时并发症较少,有利于患者术后恢复,且对于提高患者生活质量具有重要意义,值得临床推广。
[Abstract]:Objective to investigate the clinical therapeutic effect of vitrectomy for patients with proliferative diabetic retinopathy (PDR). Methods 180 eyes of 112 PDR patients were randomly divided into the observation group (58 cases, 90 eyes) and the control group (54 cases, 90 eyes), the control group was treated with photocoagulation, the observation group was treated with vitrectomy, and the treatment of two groups of patients was compared. Results after treatment, the improvement rate of stage IV in the observation group was 84.4%, the improvement rate of stage V was 64.3%, the improvement rate of phase VI was 50%, which was significantly higher than that of the control group, 27.6%, 23.3% and 16.1%, and the difference was statistically significant (P0.05); the incidence of secondary glaucoma in the observation group was 1.1%, significantly lower than that of the control group (P0.05), and the other complications occurred. There is no significant difference in the rate of comparison (P0.05). Conclusion vitrectomy has a definite effect in patients with PDR. It can effectively improve the visual acuity of the patients, at the same time, the complications are less, it is beneficial for the patients to recover after operation, and it is of great significance for improving the quality of life of the patients. It is worthy of clinical promotion.
