自体髂骨为支架的人工角膜植入重度碱烧伤兔眼的实验研究

【摘要】:目的:通过以自体髂骨组织为支架的人工角膜植入重度碱烧伤兔眼的实验,探讨自体骨组织为支架的人工角膜治疗重度碱烧伤兔眼的可行性,包括该型人工角膜的设计合理性、生物相容性、移植手术的方式以及并发症。 方法:重度碱烧伤兔眼的模型制作三个月后,将PMMA光学柱镜埋入兔髂骨三个月后,取出塑形制作成自体髂骨及骨膜组织为支架的人工角膜模型9例;将制成的人工角膜模型植入重度碱烧伤兔眼,进行实验观察。 结果:重度碱烧伤兔眼的模型制作顺利。自体髂骨为支架的人工角膜手术过程基本顺利完成,术中1例在行PMMA柱镜并周围髂骨组织取出术时发现柱镜脱离。高度怀疑柱镜在埋入后未固定牢固,导致脱离。1例术后伤口感染合并肺部感染死亡。其余8例手术顺利。在观察期内,2例人工角膜排出,余6例未见人工角膜支架溶解、房水渗漏、高眼压人工角膜排出及感染等并发症,人工角膜在眼部稳定存在3个月以上。 结论: 1、以PMMA为光学柱镜以自体髂骨为支架的人工角膜,设计基本合理,可以长期存在重度碱烧伤的兔眼,为临床治疗重度碱烧伤提供重要支持; 2、此型人工角膜移植手术方式,操作简单,术中及术后并发症相对较小; 3、通过本实验观察表明此型人工角膜具有实际临床应用的可行性,为进一步的动物实验提供理论依据及技术支持。
[Abstract]:Objective: to investigate the feasibility of using autogenous iliac tissue as scaffold in the treatment of severe alkali burn rabbit eyes, including the rationality of the design of this type of corneal artificial cornea. Biocompatibility, transplant procedures, and complications. Methods: the rabbit model of severe alkali burn was made three months later. The PMMA optical post microscope was implanted into the iliac bone of the rabbit for 3 months. The model of autogenous iliac bone and periosteal tissue was taken out and made into an artificial cornea model of 9 cases. The model of artificial cornea was implanted into the rabbit eyes of severe alkali burn, and the experimental observation was carried out. Results: the model of severe alkali burn rabbit eyes was made smoothly. The procedure of autogenous iliac graft artificial keratoplasty was basically completed successfully. One case was found to be detached from the post during the operation of PMMA and the removal of the surrounding iliac bone. Highly suspected post-endoscopy was not firmly fixed after implantation, resulting in detachment. 1 case died of postoperative wound infection with pulmonary infection. The other 8 cases were successfully operated. During the observation period, 2 cases of artificial cornea ejected, the remaining 6 cases did not see complications such as stent dissolution, aqueous humor leakage, high intraocular pressure artificial cornea excretion and infection. The artificial cornea remained stable in the eye for more than 3 months. Conclusion: 1. The design of artificial cornea with PMMA as optical pillar mirror and autogenous iliac bone as scaffold is reasonable and can provide important support for clinical treatment of severe alkali burn. 2. This type of keratoplasty is easy to operate, and has relatively small complications during and after operation. 3. The experimental results show that this type of corneal prosthesis has the feasibility of clinical application and provides theoretical basis and technical support for further animal experiments.
