TuM2-PK在鼻咽癌中的表达规律及临床意义

【摘要】:目的:探讨肿瘤M2型丙酮酸激酶(TuM2-PK)在鼻咽癌中的表达,及其与鼻咽癌浸润转移和预后的关系。 方法:应用免疫组化法检测72例鼻咽癌和15例鼻咽炎性组织中TuM2-PK的表达,并分析其与临床病理学特征及预后的关系。 结果:1、TuM2-PK在鼻咽癌组织中表达率为51.4%(37/72),显著高于鼻咽部炎性组织7%(1/15),两组有显著的统计学差异(P=0.001)。 2、TuM2-PK的表达与患者年龄、性别无明显相关(P0.05),但TuM2-PK高表达患者较低表达患者具有更高的、淋巴结转移率(P0.05)、远处转移率(P=0.047)以及复发率(P=0.035)。 3、TuM2-PK高表达组5年生存率为37.8%,TuM2-PK低表达组5年生存率为68.6%,两组有显著的统计学差异(P0.05)。单因素分析显示鼻咽癌患者5年生存率与T分期、淋巴结转移情况、远处转移、TNM分期、TuM2-PK的表达水平相关(P0.05)。多因素分析显示淋巴结转移情况、远处转移、TNM分期及TuM2-PK的表达水平为鼻咽癌患者5年生存率的影响因素(P0.05)。 结论:鼻咽癌组织中TuM2-PK表达较鼻咽炎性组织中的表达明显增强,并与鼻咽癌的T分期、淋巴结转移、远处转移以及复发密切相关。TuM2-PK可能作为判断鼻咽癌转移潜能和预后的辅助指标。
[Abstract]:Aim: to investigate the expression of M 2 type pyruvate kinase (TuM2-PK) in nasopharyngeal carcinoma (NPC) and its relationship with invasion, metastasis and prognosis of NPC. Methods: immunohistochemical method was used to detect the expression of TuM2-PK in 72 cases of nasopharyngeal carcinoma and 15 cases of nasopharyngitis, and the relationship between the expression and clinicopathological features and prognosis was analyzed. Results: 1 the expression rate of TuM2 PK in nasopharyngeal carcinoma was 51.4% (37 / 72), which was significantly higher than that in nasopharyngeal inflammatory tissue (7%, 1 / 15). There was significant difference between the two groups (P < 0.001). 2. There was no significant correlation between the expression of TuM2-PK and the age and sex of the patients (P0.05), but the patients with high expression of TuM2-PK had higher lymph node metastasis rate than those with lower expression (P0.05). Distant metastasis rate (P < 0. 047) and recurrence rate (P = 0. 035). 3. The 5-year survival rate was 37.8% in the high-expression TuM2-PK group and 68.6% in the low-expression TuM2-PK group. There was significant difference between the two groups (P0.05). Univariate analysis showed that the 5-year survival rate was correlated with T stage, lymph node metastasis, distant metastasis, TNM stage and TuM2-PK expression (P0.05). Multivariate analysis showed that lymph node metastasis, distant metastasis, TNM stage and TuM2-PK expression were related to the 5-year survival rate of NPC patients (P0.05). Conclusion: the expression of TuM2-PK in nasopharyngeal carcinoma is significantly higher than that in inflammatory tissue of nasopharynx, and it is related to T stage and lymph node metastasis of nasopharyngeal carcinoma. TuM2-competition may be used as an auxiliary index to judge metastasis potential and prognosis of nasopharyngeal carcinoma (NPC).
