鼻咽癌患者唾液、血清和外周血白细胞中EB病毒DNA的定量检测

【摘要】：目的对鼻咽癌患者唾液、血清及外周血白细胞中的EBV-DNA进行检测，分析唾液、血清、外周血白细胞中的EB病毒DNA含量与鼻咽癌的相关性。方法对40例初诊鼻咽癌患者和50例正常人的唾液、血清及外周血白细胞中的EBV-DNA进行荧光聚合酶链反应(FQ-PCR)检测。结果EBV-DNA在鼻咽癌患者血清中的检出率为67.5%，显著高于正常人血清中的EBV-DNA检出率2%(2=44.48，，P0.001)；鼻咽癌患者的唾液、外周血白细胞EBV-DNA检出率与正常人的无显著差异性；鼻咽癌患者和正常人的唾液EBV-DNA拷贝数与外周血白细胞EBV-DNA拷贝数呈正相关性；鼻咽癌患者血清EBV-DNA拷贝数与临床TNM分期有关，分期越晚含量越高。结论研究结果提示EB病毒长期存在于多数人的唾液中，也可存在于外周血B淋巴细胞中，并不能认为是一种癌前状态；EB病毒感染发生异型改变的鼻咽上皮可视为癌前状态。血清EBV-DNA检测在鼻咽癌的临床诊断中具有较高的敏感性（67.5%）和特异性（98%），有助于鼻咽癌的早期诊断；血清EBV-DNA拷贝数与临床TNM分期有关，可在分子水平对鼻咽癌的TNM分期进行补充。
[Abstract]:Aim to detect EBV-DNA in saliva, serum and peripheral blood leukocytes in patients with nasopharyngeal carcinoma (NPC), and analyze the correlation between the DNA content of EB virus in saliva, serum and peripheral blood leukocytes and nasopharyngeal carcinoma (NPC). Methods fluorescent polymerase chain reaction (FQ-PCR) was used to detect EBV-DNA in saliva, serum and peripheral blood leukocytes of 40 newly diagnosed NPC patients and 50 normal controls. Results the positive rate of EBV-DNA in serum of NPC patients was 67.5%, which was significantly higher than that of normal subjects (2%, 44.48, P0.001). There was no significant difference between the detection rate of EBV-DNA in saliva and peripheral blood leukocytes of NPC patients and normal persons, but there was a positive correlation between the EBV-DNA copy number of saliva and the EBV-DNA copy number of peripheral blood leukocytes in NPC patients and normal persons. The copy number of serum EBV-DNA in NPC patients was related to clinical TNM staging, and the later the staging, the higher the content. Conclusion the results suggest that EB virus exists in saliva and B lymphocytes of peripheral blood for a long time and can not be regarded as a precancerous state, and the nasopharynx epithelium infected with EB virus may be regarded as precancerous state. The detection of serum EBV-DNA has a high sensitivity (67.5%) and specificity (98%) in the clinical diagnosis of nasopharyngeal carcinoma, which is helpful for the early diagnosis of nasopharyngeal carcinoma. The serum EBV-DNA copy number was correlated with clinical TNM staging, and it could be used to supplement the TNM staging of nasopharyngeal carcinoma at molecular level.
