羊膜移植治疗眼化学烧伤及热烧伤的临床观察

【摘要】:目的:眼化学烧伤及热烧伤是我国常见的眼外伤,其损伤严重,传统治疗效果有限。羊膜因其独特的作用一直是眼表重建过程中研究的热点。本文观察使用羊膜移植术(Amniotic membrane transplantation,AMT)治疗眼化学烧伤、热烧伤后的临床治疗效果,以提高对这一治疗手段的认识。 方法:收集2009年09月—2011年10月我院收治的眼化学烧伤、热烧伤患者22人(22眼),其中Ⅱ度5眼,Ⅲ度14眼,Ⅳ度3眼,对其施行AMT眼表重建术,观察术后羊膜植片情况、角膜上皮修复情况、手术前后患者视力变化及术后排斥反应、角膜新生血管形成情况。术后随访6-18个月,平均12个月。 结果:术后早期羊膜植片轻度水肿,第3-5天起水肿减轻,1周后开始变薄,部分融解吸收。术后1个月,观察不到羊膜组织存在。羊膜植片于术后一周内部分脱离者10眼,其中6眼角膜上皮或基质尚存在较大范围缺损,故施行第2次羊膜移植术。22名患者的22眼中,20眼(Ⅱ度5眼,Ⅲ度14眼,Ⅳ度1眼)术后效果较好,表现为角膜刺激症状明显减轻,上皮愈合、视力有所提高,手术前后视力对比P<0.05,未出现感染或排斥现象。其中4眼(Ⅲ度3眼,,Ⅳ度1眼)在首次羊膜移植后,因植片脱落、融解,眼表修复效果不满意,经历2次手术。2眼(Ⅳ度2眼)手术失败,表现为经过2次AMT,仍出现角膜穿孔,最终行板层角膜移植术。最终共12眼(Ⅲ度9眼,Ⅳ度3眼)发生角膜新生血管:<1个象限者5眼(Ⅲ度5眼),1-3象限者5眼(Ⅲ度4眼,Ⅳ度1眼),>3个象限者2眼(Ⅳ度2眼)。 结论:1、对于中、重度眼化学烧伤、热烧伤病例,AMT可以起到一定减轻症状,缩短病程,优化预后的效果。对于极严重者可优化眼表微环境,为进一步手术争取时间。2、AMT术后早期羊膜脱落、融解,临床效果不满意时,可考虑多次移植以取得较稳定的临床效果。3、在重度病例,单纯靠AMT无法完成眼表重建的全部过程。应把AMT看做眼表重建的一个步骤,待炎症反应减轻,眼表环境好转,需根据损伤情况进一步处置,以完成损伤后的眼表重建。
