应用FD-OCT检测正常及近视性儿童视网膜神经纤维层的厚度

【摘要】:目的应用RTVue傅里叶域光学相干断层扫描(Fourier-domain Optical coherencetomography,FD-OCT)检测正常及不同屈光度儿童的视网膜神经纤维层(Retinal nerve fiberlayer,RNFL)厚度,探讨不同屈光儿童RNFL厚度的分布特点,屈光度和眼轴改变与RNFL厚度的关系。 方法对4-15岁儿童(59例男27例40只眼,女32例43只眼)83只眼近视组,其中低度近视组26只眼、中度近视组18只眼、高度近视组17只眼,及22只眼正常对照组,使用美国Optovue公司生产的RTVue-FD-OCT分别检测平均RNFL厚度和8个区域(TU,ST,SN,NU,NL,IN,IT,TL)的RNFL厚度,采用SPSS16.0软件进行统计分析RNFL厚度分布特点以,采用散点图分析屈光度和眼轴与RNFL厚度的线性关系。 结果正常组平均RNFL厚度:112.77±10.30μm,视盘周围8个区域(TU,ST,SN,NU,NL,IN,IT,TL)的厚度曲线呈双驼峰分布,厚度分别为94.13±18.99μm,157.08±22.28μm,157.08±22.28μm,131.50±20.77μm,70.34±13.27μm,65.34±13.91μm,129.71±20.98μm,166.82±20.11μm,88.00±16.95μm,112.87±10.38μm。各近视组平均RNFL厚度分别为:低度近视组106.77±8.60μm,中度近视组101.37±8.88μm,高度近视组85.64±10.58μm,正常对照组与近视组间统计学差异(F=22.860,P=0.000);八个区域中,除TU(F=1.394,P=0.252)及TL(F=2.337,P=0.081)外,其余6区域组间均有统计学意义(F=23.177,P=0.000;F=11.778,P=0.000;F=8.573,P=0.000,F=5.813,P=0.001,,F=5.139,P=0.003,F=11.091,P=0.000,F=22.860,P=0.000);平均RNFL厚度与屈光度之间关系:Y=-3.762x+115.3,R=-0.787,P㩳0.05;平均RNFL厚度与眼轴长度之间关系:Y=-6.886x+268.4, R=-0.798,P㩳0.05。 结论正常及屈光不正儿童RNFL厚度曲线均呈双驼峰分布,以上方及下方为最厚波峰,鼻侧及颞侧最薄为波谷。儿童平均RNFL厚度与屈光度及眼轴长度呈负相关,随屈光度数及眼轴的增加而变薄。
[Abstract]:Objective to detect the thickness of retinal nerve fiber layer (Retinal nerve fiberlayer,RNFL) in children with normal and different diopter by RTVue Fourier domain optical coherence tomography (Fourier-domain Optical coherencetomography,FD-OCT), and to explore the distribution characteristics of RNFL thickness in children with different refractive acuity and the relationship between diopter and axial changes and RNFL thickness. Methods 83 eyes of myopia group (59 males, 27 eyes, 40 eyes, female 32 cases, 43 eyes), including 26 eyes in low myopia group, 18 eyes in moderate myopia group, 17 eyes in high myopia group and 22 eyes in normal control group, were used to detect the average RNFL thickness and the RNFL thickness of 8 regions (TU,ST,SN,NU,NL,IN,IT,TL) produced by Optovue Company in the United States. The distribution characteristics of RNFL thickness were statistically analyzed by SPSS16.0 software, and the linear relationship between diopter and eye axis and RNFL thickness was analyzed by scatter plot. Results in the normal group, the average thickness of RNFL was 112.77 卤10.30 渭 m, and the thickness curve of 8 regions around the optic disc (TU,ST,SN,NU,NL,IN,IT,TL) was double hump distribution, the thickness was 94.13 卤18.99 渭 m, 157.08 卤22.28 渭 m, 131.50 卤20.77 渭 m, 70.34 卤13.27 渭 m, 65.34 卤13.91 渭 m, 129.71 卤20.98 渭 m, 166.82 卤20.11 渭 m, 88.00 卤16.95 渭 m, 112.87 卤10.38 渭 m, respectively. The average RNFL thickness of each myopia group was 106.77 卤8.60 渭 m in low myopia group, 101.37 卤8.88 渭 m in moderate myopia group, 85.64 卤10.58 渭 m in high myopia group, and there was significant difference between normal control group and myopia group (F 鈮
