感音性聋实施畸变产物耳声发射测试的参数设置

【摘要】：目的探讨感音性聋实施畸变产物耳声发射(DPOAE)测试的参数设置。方法随机选取2011年3月至2014年10月上海市崇明县第二人民医院收治的34例(68耳)感音性聋患者作为感音性聋组,另选取同期本院进行体检的34例(68耳)听力正常的健康者作为对照组。对照组未进行任何干预,同时对两组受试者进行感音性聋患者DPOAE测试。比较两组受试者的最佳参数设置并进行统计分析。结果感音性聋组L1/L2=65/50 d BSPL时2 k Hz的DPOAE引出率为98.5%(67/68),低于对照组的100.0%(68/68),差异有统计学意义(P0.05)。感音性聋组患者L1/L2=70/70、65/50 d BSPL时0.5、1、2 k Hz的DPOAE反应幅值均显著低于对照组,差异有统计学意义(P0.05)。L1/L2=70/70 d BSPL时0.5、1、2 k Hz的灵敏度分别为82.4%(56/68)、75.0%(51/68)、73.5%(50/68),特异度分别为85.3%(58/68)、66.2%(45/68)、70.6%(48/68),接受者操作特征曲线(ROC曲线)下面积分别为0.822、0.841、0.804;ROC曲线下面积分别为0.981、0.981、0.993。L1/L2=65/50 d BSPL时0.5、1、2 k Hz的灵敏度和特异度均高于L1/L2=70/70 d BSPL。结论感音性聋实施DPOAE测试的刺激强度应该设置为L1/L2=65/50 d BSPL。
[Abstract]:Objective to investigate the parameter setting of distortion product otoacoustic emission (DPOAE) test in sensorineural deafness. Methods from March 2011 to October 2014, 34 patients (68 ears) with sensorineural deafness were randomly selected as the group of sensorineural deafness, and 34 healthy patients (68 ears) with normal hearing were selected as the control group. No intervention was performed in the control group, and DPOAE test was performed in both groups of subjects with sensorineural deafness.
