内镜逆行胰胆管造影术中运用导丝测量法与支架测量器测量法选择支架的临床观察

【摘要】:目的探讨导丝测量法与支架测量器测量法在内镜逆行胰胆管造影术(ERCP)中选择合适的植入支架的应用中的临床效果。方法收集该院2013年1月-2015年9月确诊为恶性胆道梗阻、胆管结石、胆源性胰腺炎和胰管结石的患者55例进行两种测量方法的随机对照观察。对两组患者的测量值、实际值、误差值、更换情况及测量时间的数据进行比较。结果两组支架放置更换率比较差异无统计学意义(P0.05),但导丝组的更换率较支架测量器组的更换率更高(14.81%vs 3.57%)。单纯对导丝组和支架测量器组的测量值和实际值进行比较,两者间差异均无统计学意义(P0.05)。通过测量值和实际值之间的差值来判断导丝和支架测量器的准确度,支架测量器组明显优于导丝组,两者之间的比较差异有统计学意义(P0.05)。两组方法得出测量结果的时间的比较,支架测量器组明显优于导丝组,两者之间比较差异有统计学意义(P0.05)。结论支架测量器测量法较导丝测量法而言具有方便、快捷、准确度高、支架更换率低的特点。
[Abstract]:Objective to investigate the clinical effect of wire guide and stent measurement in the selection of appropriate stent implantation in endoscopic retrograde cholangiopancreatography (ERCP).Methods from January 2013 to September 2015, 55 patients with malignant biliary obstruction, cholelithiasis, biliary pancreatitis and pancreatic duct stone were studied.The measured value, actual value, error value, replacement condition and measurement time of the two groups were compared.Results there was no significant difference between the two groups in the replacement rate of stent placement, but the replacement rate of the wire guide group was higher than that of the stent measuring apparatus group (14.81% vs 3.57%).There was no significant difference between the measured values and the actual values of wire guide group and scaffold group.The accuracy of the wire guide and the scaffold was judged by the difference between the measured value and the actual value. The scaffold group was obviously superior to the wire guide group, and the difference between the two groups was statistically significant (P 0.05).The comparison of time between the two groups showed that the scaffold group was significantly better than the wire guide group, and the difference between the two groups was statistically significant (P 0.05).Conclusion compared with the wire guide method, the scaffold measuring method has the advantages of convenience, rapidity, high accuracy and low replacement rate.
