急性胰腺炎患者血浆穿透素水平表达及临床意义

【摘要】：目的探讨急性胰腺炎(AP)患者血浆穿透素(PTX)3水平的表达及临床意义。方法 97例AP患者为研究组,按病情严重程度分为重度组(n=31)、中度组(n=32)和轻度组(n=34),另随机选取80例健康体检者作为对照组。采用酶联免疫法(ELISA)分别检测对照组入院第1天和AP患者入院第1、4、7天血清中PTX3水平。采用急性生理与慢性健康评分(APACHE)Ⅱ、严重程度床边指数评分(BISAP)、Ranson评分评估病情严重程度,并分析血清中PTX3水平与以上评分的相关性。结果研究组入院第1天PTX3水平明显高于对照组(P0.05);不同程度AP患者入院第1、4、7天组间PTX3水平比较均有统计学差异(均P0.05),以重度组为最高,中度组次之,轻度组最低(P0.05);随着时间的推移,不同程度AP患者PTX3水平均呈下降趋势,入院第7天与第1天比较差异显著(P0.05)。根据Pearson相关分析结果显示,PTX3水平与APACHEⅡ评分、BISAP评分、Ranson评分均呈显著正相关(r=0.883、0.832、0.749,均P0.05)。结论 PTX3水平可较好地反映AP患者病情进展,可作为AP预后的预测指标。
[Abstract]:Objective to investigate the expression and clinical significance of plasma penetin (PTX) 3 in patients with acute pancreatitis (AP).Methods 97 patients with AP were divided into two groups according to the severity of the disease. They were divided into severe group (31 cases) and mild group (32 cases) and mild group (n = 34). Another 80 healthy persons were randomly selected as control group.The serum PTX3 levels in the control group on the first day of admission and the AP patient on the 4th day of admission were detected by enzyme linked immunosorbent assay (Elisa).Acute physiological and chronic health scores were used to evaluate the severity of the disease. The severity index score (BISAP / Ranson score) was used to evaluate the severity of the disease, and the correlation between the serum PTX3 level and the above scores was analyzed.Results the level of PTX3 in the study group was significantly higher than that in the control group on the first day of admission, and the level of PTX3 in the patients with different degrees of AP was significantly higher than that in the control group on the 7th day after admission (P 0.05), the highest in the severe group and the second in the moderate group.With the passage of time, the PTX3 level of AP patients with different degrees showed a downward trend, and there was a significant difference between the 7th day of admission and the first day of admission.According to the results of Pearson correlation analysis, there was a significant positive correlation between the level of PTX3 and the APACHE 鈪，
