超声造影对脂肪缺失性非均匀脂肪肝临床应用价值

【摘要】:目的:通过超声造影分析非均匀脂肪肝内脂肪缺失处病灶的血流灌注特征,进一步探讨超声造影对脂肪缺失性非均匀脂肪肝临床应用价值。 方法:2012年3月~2014年3月选取在我院门诊或住院就诊时常规超声检查提示肝脏局部脂肪缺失的患者,并常规超声不能确定其有无占位性病变,给予超声造影检查。 结果:30例患者共45个脂肪缺失处,17脂肪缺失处位于肝右前叶近肝表面处,2处脂肪缺失处位于肝右后叶,2处脂肪缺失位于左外叶下段肝脏边缘处,其余24处脂肪缺失均位于胆囊窝处及门静脉供血附近,根据穿刺活检病理证实,45例脂肪缺失经超声造影造影诊断3例为转移癌,,2例原发性肝癌,2例胆管细胞性肝癌,2例局灶结节增生,11例肝血管瘤,15个单纯脂肪缺失。常规超声在45个脂肪肝内脂肪缺失处病灶正确诊断病灶类型25个(55.6%),超声造影后正确诊断病灶类型42个(93.3%)。肿瘤周围脂肪缺失区的形成可能与病灶对肝实质的压迫以及该肿瘤有丰富动脉供血引起前方区域的门静脉血流相对减少有关。 结论:超声造影能够反映脂肪缺失性非均匀脂肪肝内血流灌注特征,提高了超声对脂肪缺失区定性诊断能力,当不能确定为脂肪缺失处低回声内是否有肿瘤时,,为避免延误治疗须尽早定性,及时进行超声造影检查。
[Abstract]:Objective : To investigate the clinical value of ultrasound contrast imaging in the clinical application of fat loss non - uniform fatty liver by ultrasound contrast analysis .
Methods : In March 2012 ~ March 2014 , the patients with local fat loss in the liver were indicated by routine ultrasound examination at the outpatient or in - hospital visit of our hospital , and the conventional ultrasound was unable to determine the presence or absence of occupying lesions , and the ultrasound contrast examination was given .
Results : There were 45 fat deletions in 30 patients , 17 fat deletions at the proximal hepatic surface of the right anterior lobe of the liver , 2 fat loss in the right posterior lobe of the liver , 2 cases of primary liver cancer , 2 bile duct cell liver cancer , 2 focal nodular hyperplasia , 11 cases of hepatic hemangioma , and 15 simple fat loss .
Conclusion : Contrast - enhanced ultrasonography can reflect the characteristics of blood perfusion in fat - deficient non - uniform fatty liver , and improve the qualitative diagnosis ability of fat loss region . When it is not possible to determine whether there is tumor in low echo of fat loss , the ultrasonic contrast examination should be performed as soon as possible in order to avoid delay treatment .
