急性胰腺炎患者血浆中性粒细胞明胶酶相关脂质运载蛋白水平变化及意义

【摘要】：目的探讨急性胰腺炎患者血浆中性粒细胞明胶酶相关脂质运载蛋白(NGAL)水平变化及临床意义。方法选取60例急性胰腺炎患者为观察组,其中轻症急性胰腺炎(MAP)28例、重症急性胰腺炎(SAP)32例;常规治疗14天,治疗有效48例,治疗无效12例(均为SAP)。选取同期健康体检者35例为对照组。检测观察组及对照组血浆NGAL水平(ELISA法检测)及中性粒细胞NF-κB mRNA相对表达量(RT-PCR法检测),分析急性胰腺炎患者NGAL水平与NF-κB mRNA表达的相关性。结果观察组血浆NGAL水平及中性粒细胞NF-κB mRNA相对表达量均高于对照组(P均0.01),且SAP患者均高于MAP患者(P均0.05)。治疗无效者入院时血浆NGAL水平及中性粒细胞NF-κB mRNA相对表达量均高于有效者(P均0.01)。急性胰腺炎患者NGAL活性与NF-κB mRNA表达呈正相关(r=0.156,P0.05)。结论急性胰腺炎患者血浆NGAL水平升高,其水平变化参与急性胰腺炎的发生、发展,激活NF-κB通路、促进炎症反应可能是其作用机制。
[Abstract]:Objective to investigate the changes and clinical significance of plasma neutrophil gelatinase-associated lipid carrier protein (NGAL) in patients with acute pancreatitis. Methods Sixty patients with acute pancreatitis were selected as the observation group, including 28 patients with mild acute pancreatitis (map) and 32 patients with severe acute pancreatitis (SAP). 35 healthy persons in the same period were selected as the control group. The plasma NGAL level (Elisa) and the relative expression of NF- 魏 B mRNA in neutrophils (RT-PCR) were detected in the observation group and control group. The correlation between NGAL level and NF- 魏 B mRNA expression in acute pancreatitis was analyzed. Results the plasma NGAL level and the relative expression of NF- 魏 B mRNA in neutrophils in the observation group were higher than those in the control group (P0.01), and the levels of SAP patients were higher than those in the map patients (P 0.05). The plasma NGAL level and the relative expression of NF- 魏 B mRNA in neutrophils were significantly higher in the ineffective patients than in the effective ones (P 0.01). There was a positive correlation between NGAL activity and NF- 魏 B mRNA expression in patients with acute pancreatitis (P 0.05). Conclusion the plasma NGAL level in patients with acute pancreatitis is increased. The changes of NGAL level may be involved in the occurrence and development of acute pancreatitis, activation of NF- 魏 B pathway and promotion of inflammatory response.
