肝硬化患者的吲哚菁绿清除试验和MELD评分相关性分析研究

【摘要】:目的:研究肝硬化患者吲哚菁绿清除试验与MELD评分之间关系。ICG清除试验中清除率(K)和肝内15分钟内保留率(R15)是评估患者肝脏功能的敏感指标,终末期肝病模型(MELD)评分是评估等待肝移植患者死亡率的有效方法,已广泛应用于临床。本研究通过回顾性分析研究肝硬化患者清除率试验(K值、R15值)、MELD评分与Child-Pugh评分之间的相关性及其临床意义。方法:回顾性分析2012年6月至2013年6月共78例被我科诊断为肝硬化患者,根据Child-Pugh分级分为A、B、C三组,ICG试验中K值和R15值是由脉冲分光光度法(DDG-3300K)测出,MELD评分是根据MELD评分公式计算得出。A、B、C三组组间采用两独立样本t检验,ICG清除试验和MELD评分之间采用spearman相关性分析。结果:患者在A、B、C三组中K值、R15及MELD评分各组间经t检验有显着的统计学差异。K值在A组中最大,为0.15±0.06,C组中最小,为0.07±0.04。R15在C组中最大,为38.1±15.6,在A组中最小12.5±10.2,MELD评分在C组中最大,为12.5±5.6,A组中最小,为3.9±3.7。各组间采用两样本t检验,在A组、B组与C组比较中,k值、R15、MELD评分各组均有统计学意义(P0.05)。ICG清除试验(K值和R15)与MELD评分之间经直线相关性分析有着显著的相关性,K值与MELD评分之间呈负相关(r=-0.443,P0.01),而R15和MELD评分之间呈正相关(r=0.478,P0.01),K值与R15之间呈负相关性(r=-0.666,P0.01)。K值、R15、MELD评分与Child-Pugh评分之间经直线相关性分析示:Child-Pugh评分与K值、R15、MELD评分之间存在显著的相关性,它与K值呈负相关(r=Child-Pugh评分-0.599,P0.01),与R15呈正相关(r=Child-Pugh评分0.475,P0.01),与MELD评分亦呈正相关(r=Child-Pugh评分0.626,P0.01)。结论:根据Child-Pugh评分分组,ICG清除试验(K值和R15)与MELD评分之间存在显著相关性,其中R15值,可能是评价肝硬化患者肝功能合适的指标。
[Abstract]:Objective: to study the relationship between indocyanine green clearance test and MELD score in patients with liver cirrhosis. The clearance rate (K) and intrahepatic retention rate (R15) in ICG clearance test were sensitive indexes for evaluating liver function in patients with liver cirrhosis. The (MELD) score of end-stage liver disease model is an effective method to evaluate the mortality of patients waiting for liver transplantation and has been widely used in clinic. In this study, the correlation and clinical significance of clearance test (K value, R15), MELD score and Child-Pugh score) in patients with liver cirrhosis were analyzed retrospectively. Methods: from June 2012 to June 2013, 78 patients with liver cirrhosis diagnosed by our department were divided into three groups according to Child-Pugh classification: group A, group B, group C. The values of K and R15 in ICG test were determined by pulse spectrophotometry (DDG-3300K). The MELD score was calculated according to the MELD score formula. A, B, C groups were tested by two independent samples t test, ICG clearance test and MELD score were analyzed by spearman correlation analysis. Results: in group A, B, C, the scores of K, R15 and MELD were significantly different by t test. K value was the largest in group A (0. 15 卤0. 06), the smallest in group C (0. 07 卤0.04.R15), the largest in group C (0. 15 卤0. 06), and the lowest in group C (0. 07 卤0. 06), the highest in group C (0. 15 卤0. 06). It was 38.1 卤15.6 in group A, 12.5 卤10.2 in group A, and 12.5 卤5.6 in group C and 3.9 卤3.7 in group A, respectively. In group A, group B and group C, the value of k, R15, and so on, were compared with each other by two-sample t-test. There was a significant correlation between MELD clearance test (K value and R15) and MELD score (P 0.05). There was a negative correlation between K value and MELD score (r = 0.443, P0.01). There was a positive correlation between R15 and MELD score (r = 0.478, P0.01), and a negative correlation between K value and R15 score (r = 0.666, P0.01). The linear correlation analysis between MELD score and Child-Pugh score showed that there was a significant correlation between Child-Pugh score and K value, R15, meld score, and it was negatively correlated with K value (r=Child-Pugh score-0.599, P0.01). There was a positive correlation between R15 and R15 (r=Child-Pugh score 0.475, P0.01) and MELD score (r=Child-Pugh score 0.626, P0.01). Conclusion: according to Child-Pugh score, there is a significant correlation between ICG clearance test (K value and R15) and MELD score, in which R15 value may be an appropriate index to evaluate liver function in patients with liver cirrhosis.
