经皮置管引流对重症急性胰腺炎胰腺损伤影响的实验研究

研究背景和研究目的重症急性胰腺炎(Severe acute pancreatitis,SAP)是消化系统的常见病,该病在发病过程中常以胰腺坏死、胰周积液且并发多器官功能衰竭为其主要特点。临床研究表明,大多数SAP患者在起病早期即可产生大量富含胰酶、促炎细胞因子等有害物质的胰腺炎相关性腹水。胰腺炎相关性腹水形成的主要原因有:(1)因胰管压力增高、胆汁返流、胰酶激活并释放入细胞间质等原因导致胰腺组织损伤、溶解,形成大量富含胰酶的组织液从胰腺渗出;(2)胰腺组织中浸润的中性粒细胞被激活产生大量促炎细胞因子使得无菌性炎症发生,引起血管内皮损伤导致毛细血管通透性增加。大量胰腺炎相关性腹水的形成可引起机体有效循环血量下降,造成机体多脏器供血不足,同时腹水中的有害成分不仅对浸泡在腹水中的腹腔内脏器引起直接损伤,也可以经腹膜吸收进入体循环对远隔器官产生损伤作用。可见,在SAP发病早期若能及时、有效地将胰腺炎相关性腹水引流至体外,可能对SAP患者的治疗有重要的作用。基于以上认识,本研究以Wistar大鼠作为实验对象,在建立SAP大鼠模型的基础上,将大鼠腹腔内胰腺炎相关性腹水引流至体外,观察经皮腹腔置管引...
