白内障术后眼内炎54例的回顾性临床分析

【摘要】：目的：研究白内障术后眼内炎（endophthalmitis after cataract surgery）的临床特点、致病菌、治疗及预后。 方法：对2003年1月至2013年12月期间广西医科大学第一附属医院眼科收治的54例（55眼）白内障术后眼内炎患者的临床资料进行非对照性回顾性分析，主要分析其发病特点、治疗情况、病原菌的检查结果、治疗结果。 结果：平均发病时间为白内障术后7.29天,42眼（76.36%）发病时间在7天内，49眼（89.09%）发病时间在白内障术后2周内，6眼（10.91%）发病时间大于白内障术后2周。41例于我院行前房液或玻璃体液病原学检查，阳性为11例，，房水及玻璃体病原体检出率为26.83％(11/41)。除1例患者视力检查不合作外,53例54眼中，治疗前裸眼视力≥0.05有5例，占9.25%(5/54)，治疗后裸眼视力≥0.05的有27例，占50.00%(27/54)，治疗后总体裸眼视力好于治疗前(p＜0.05)。无1例摘除眼球。 结论：早期诊断并尽早合理的进行相关干预可不同程度的挽回患者部分视功能。不同白内障手术切口术后眼内炎的发生率无差异。围手术期是否采用静脉和结膜下使用抗生素，其白内障术后眼内炎的发生率无差异。前房液或玻璃体液培养出的致病菌中，以表皮葡萄球菌为代表的凝固酶阴性葡萄球菌为主。
[Abstract]:Objective: to study the endophthalmitis after cataract after cataract surgery. Surgery. clinical features, pathogenic bacteria, treatment and prognosis. Methods: from January 2003 to December 2013, 54 cases (55 eyes) of ophthalmology in the first affiliated Hospital of Guangxi Medical University were treated. The clinical data of patients with endophthalmitis after cataract surgery were analyzed retrospectively. The main analysis of its disease characteristics, treatment, pathogen detection results, treatment results. Results: the average onset time was 7.29 days (42 eyes, 76.3636) within 7 days, 49 eyes (89.09 eyes) occurred within 2 weeks after cataract surgery. The etiology of anterior chamber fluid or vitreous fluid was detected in 41 cases in our hospital 2 weeks after cataract. 11 cases were positive.
