【摘要】:目的:甲状腺相关眼病(TAO)主要累及眶脂肪和眼外肌,准确的眼眶影像学检查对于TAO的诊疗非常重要。本文主要探讨通过眼眶冠状位CT图像定量测量TAO患者眼外肌横截面积的可行性,评价其临床意义,并根据眼外肌情况讨论TAO亚型的分类。 方法:回顾性病例观察研究。本研究主要纳入20例正常对照患者,42例TAO住院患者眼眶冠状位CT平扫影像学资料,利用autoCAD软件计算各眼外肌最粗处横截面积。统计TAO患者各条眼外肌增粗情况,分析眼外肌总横截面积与CAS评分、眼球突出度、眼压、矫正视力的相关性,并且根据眼外肌总横截面和眼球突出度,将TAO分为四种亚型:Ⅰ型:眶脂肪和眼外肌均不增粗;Ⅱ型:眶脂肪增多为主;Ⅲ型:眼外肌增粗为主;Ⅳ型:眶脂肪增多、眼外肌增粗。 结果: TAO组患者下直肌、外直肌、提上睑肌上直肌复合体、内直肌及眼外肌总横截面积均大于正常对照组,差异有统计学意义(p<0.001,p=0.033,,p<0.001,p<0.001,p<0.001)。下直肌、提上睑肌上直肌复合体增粗最常见(分别为60.71%、58.33%),其次为内直肌(40.48%),外直肌增粗较为少见(25%),眼外肌总横截面积增大(66.67%)。眼外肌总横截面积与CAS评分成正相关(r=0.791,p<0.001),与矫正视力成负相关(r=-0.658,p<0.001),与眼压成正相关(r=0.454,p=0.019),提上睑肌上直肌复合体横截面积与上睑退缩程度成正相关(r=0.304,p=0.005)。根据眼外肌总横截面积和眼球突出度,可将TAO分为4种亚型:Ⅰ型0例,眶脂肪和眼外肌均不增多;Ⅱ型9例(21.4%),眶脂肪增多为主;Ⅲ型10例(23.8%),眼外肌增粗为主;Ⅳ型23例(54.8%),眶脂肪和眼外肌均增多。Ⅱ型患者的年龄小于Ⅲ型和Ⅳ型TAO,差异有统计学意义(p<0.001)。Ⅱ型患者CAS评分均小于4分,处于非活动期,视力一般不受影响,多有131I治疗史,而Ⅲ型和Ⅳ型患者CAS评分明显高于Ⅱ型患者,矫正视力低于Ⅱ型患者,容易发展为视力威胁型TAO。 结论: TAO患者眼眶CT冠状位平扫定量测量眼外肌横截面积,能够准确反映各眼外肌增粗的情况,眼外肌总横截面积的大小可用于评估TAO患者的活动性,并为区分TAO亚型提供了可靠依据,在TAO亚型中,Ⅱ型患者以眶脂肪增多为主,多发生于年轻女性,有131I治疗史,多双眼发病,无复视,病情处于稳定期;Ⅲ型和Ⅳ型患者以眼外肌增粗为主,有部分患者单眼发病,复视较明显,病情较重。
[Abstract]:Objective: thyroid associated ophthalmopathy (TAO) is mainly involved in orbital adipose tissue and extraocular muscle and orbital image accurate examination is very important for the diagnosis and treatment of TAO. This paper mainly discusses the feasibility of extraocular orbital coronal CT image quantitative measurement of TAO in patients with muscle cross-sectional area, to evaluate its clinical significance, and according to the discussion of extraocular muscle the classification of subtype TAO.
Methods: a retrospective case study. This study included 20 cases of normal control patients, 42 cases of TAO orbital coronal CT scan imaging data, using autoCAD software to calculate the extraocular muscles at the thickest cross-sectional area. The statistical TAO patients with extraocular muscle enlargement of extraocular muscle total analysis the cross-sectional area of the CAS score, the degree of exophthalmos, intraocular pressure, visual acuity and correlation, according to the total cross section of extraocular muscle and exophthalmos, TAO is divided into four subtypes: type I: orbital fat and extraocular muscles were not thickened; type II: orbital fat; type III: extraocular muscle enlargement; type IV: orbital fat, extraocular muscle.
