角膜最大曲率方向切口与传统术式白内障超声乳化术后角膜散光的对比研究

【摘要】：目的:对比最大角膜屈光力子午线轴位的透明角膜切口与传统白内障超声乳化术后角膜散光的变化。方法:将58例(60只眼)白内障患者随机分为A、B两组。A组从最大角膜屈光力子午线轴位做3.0mm透明角膜切口,B组从传统颞上侧(右眼)或鼻上侧(左眼)行3.0 mm透明角膜切口。两组均行白内障超声乳化吸除术联合可折叠式人工晶体植入术。分别于术前及术后1 d、术后1周、术后1个月用CT200角膜地形图仪测量角膜散光度和轴向并计算手术源性散光度。比较两组术后不同时间的角膜散光度、手术源性散光度和裸眼视力。结果:A、B两组术前和术后各阶段视力比较差异无统计学意义(均P0.05)。两组术后1周和术后1个月平均角膜散光度A组B组,差异有统计学意义(均P0.05)。术后1周和术后1个月角膜散光的矢量分析A组B组,差异均有统计学意义(均P0.05)。结论:相对于传统手术切口,白内障手术中选择角膜最大曲率方向做透明角膜切口,能够使患者术后的角膜散光度和手术源性散光度减小。
[Abstract]:Objective: To compare the changes of corneal astigmatism corneal refractive power maximum radial axis of the transparent corneal incision and traditional cataract after phacoemulsification. Methods: 58 cases (60 eyes) of cataract patients were randomly divided into A, B two group.A group from the maximum corneal refractive power radial axis 3.0mm transparent corneal incision, group B from the traditional temporal side (right eye) or nasal side (left) for 3 mm clear corneal incision. The two groups underwent phacoemulsification and foldable IOL implantation. Before and after surgery to 1 D, after 1 weeks, 1 months after operation with CT200 angle membrane topography measurement of corneal astigmatism and axial and the calculation of surgically induced astigmatism degree. The two groups were compared after different time of corneal astigmatism, surgically induced astigmatism and uncorrected visual acuity. Results: A, B two groups had no statistical significance before and after surgery in each stage of visual acuity difference (P0.05) for 1 weeks. The two groups after operation And 1 months after surgery, the average corneal astigmatism of group A B group, the difference was statistically significant (P0.05). After 1 week and 1 months vector of corneal astigmatism of group A B group, the differences were statistically significant (P0.05). Conclusion: compared with traditional surgical incision cataract surgery in selected choose the maximum corneal curvature direction of clear corneal incision, can make patients with postoperative corneal astigmatism and surgical astigmatism is reduced.
