折射衍射结合型多焦点人工晶体-Acrysof ResToR植入术后的视功能研究

【摘要】: 目的通过比较白内障超声乳化联合植入Acrysof ResToR及Acrysof Natural.Array SA-40N术后人工晶状体眼(IOL)的视功能状态,评价Acrysof Re sToR多焦点人工晶状体(MIOL)的临床效果。 方法随机化临床试验,选择合适的年龄相关性白内障患者,其中ReSTOR组:30例36眼;Natural组:28例30眼,;Array组50例60眼。观察三组患者术后3月视力,包括未矫正远视力(UCDVA)、未矫正近视力(UCNVA)、最佳矫正远视力(BCDVA)、最佳矫正近视力(BCNVA)、远矫正下的近视力(DCNVA)、中间距离视力(40cm,60cm,80cm),对比敏感度(CS)、焦点深度,并进行关于视觉症状、戴镜率和满意度的问卷调查。 结果术后3月,ResToR组在UCDVA.BCDVA.BCNVA和Natural组Array组无显著差异(P0.05),但在UCNVA,DCNVA优于Natural、Array(P0.05)。关于中间距离视力,ResToR和Array无显著性差异(P0.05),明显优于Natural组(P0.05)。ResToR在术后屈光状态(球镜、柱镜)和Natural.Array均无差异。在焦点深度方面,ReSTOR和Array,它们的焦点深度范围无差异,且波峰、波谷出现的位点基本一致。ReSTOR的焦点深度范围5.88D,Natural的焦点深度范围1.88D,有统计学意义(P0.05).对比敏感度方面,ReSTOR和Array在各个空间频段无统计学差异(P0.05)。ReSTOR组各空间频段均低于Natural组,但是除了高空间频段(18cpd)外,其余差异无显著意义(P0.05)。ReSTOR和Array植入术后的视觉症状,均以眩光和光晕为主,两者无显著性差异。ReSTOR组患者术后受眩光、光晕影响较Natural组明显(P0.05),但均可忍受。ReSTOR组视近时“戴镜率”低于Natural组,差异有统计学意义(P0.05)。ReSTOR和Array“戴镜率”差异无显著意义(P0.05)。 结论:新型衍射型多焦点人工晶体ReSTOR能提供良好的全程视力,有效减低患者对老视镜的依赖,提高白内障患者术后的视觉质量。
[Abstract]:Objective to compare the effect of phacoemulsification combined with implantation of Acrysof ResToR and Acrysof Natural.Array SA-40N on intraocular lens (IOL). IOL). To evaluate the clinical effect of Acrysof re sToR multifocal intraocular lens. Methods A randomized clinical trial was conducted to select suitable age-related cataract patients, including 30 cases (36 eyes) in ReSTOR group. Natural group: 28 cases (30 eyes); The visual acuity of the three groups was observed on March, including UCDVAV without correction and UCNVA without corrected near visual acuity (UCNVA) in 50 patients (60 eyes) in Array group. The best corrected distant visual acuity (BCDVAV), the best corrected near visual acuity (BCNVAV), the near visual acuity (DCNVAV) and the intermediate distance visual acuity (40 cm ~ 60 cm ~ (80 cm)). Contrast sensitivity, depth of focus, and a questionnaire on visual symptoms, wear rate, and satisfaction. Results on March, there was no significant difference between the UCDVA.BCDVA.BCNVA group and the Natural group in Array group (P 0.05), but in UCNVA. DCNVA was better than that of natural Arraya (P 0.05). There was no significant difference between ResToR and Array in the middle distance visual acuity (P 0.05). Compared with Natural group, there was no significant difference in refractive state (spherical, cylindrical) and Natural.Array, but in focal depth. The focal depth range of ReSTOR and Arrayhas no difference, and the peaks and troughs are basically consistent with the focal depth range of .ReSTOR (5.88D). The focal depth range of Natural was 1.88D, which was statistically significant (P 0.05). Contrast sensitivity. There was no statistical difference between ReSTOR and Array in each spatial frequency band. The spatial frequency of ReSTOR and Array group was lower than that of Natural group. But with the exception of high spatial frequency band (18cpdd), there was no significant difference in visual symptoms after P0.05 or Array implantation. Glare and halo were the main visual symptoms. There was no significant difference between the two groups. The patients in the rest group suffered from glare and the effect of halo was significantly higher than that in the Natural group (P 0.05). However, the "wear rate" in the. Rest group was lower than that in the Natural group. The difference was statistically significant (P 0.05). There was no significant difference between Array (P 0.05) and Array (P 0.05). Conclusion: the new diffractive multi-focus intraocular lens (ReSTOR) can provide good visual acuity in the whole process, reduce the dependence of the patients on the presbyopia, and improve the visual quality of the cataract patients after operation.
