角蛋白19表达与鼻咽癌临床病理特征的关系研究

【摘要】:目的研究角蛋白19(Cytokeratin 19或Keratin 19, CK19)在鼻咽癌组织和细胞中的表达情况,分析其表达与鼻咽癌临床病理特征的关系。 方法62例鼻咽癌、17例淋巴结转移性鼻咽癌和16例慢性鼻咽炎(作为正常鼻咽粘膜对照组)标本均由病理证实,采用免疫组化S-P法检测其中CK19的表达水平,并分析其表达水平与鼻咽癌临床病理特征的关系;采用western blot检测不同分化程度和不同转移潜能NPC细胞系中CK19蛋白的表达情况。 结果 1)免疫组化结果显示CK19主要表达在细胞胞浆和胞膜,鼻咽癌CK19阳性率为74.2%(46/62);淋巴结转移性鼻咽癌中阳性率为88.2%(15/17);对照组正常鼻咽上皮中阳性率为12.5%(2/16);CK19表达在淋巴结转移性鼻咽癌高于鼻咽癌标本,在鼻咽癌中高于正常对照组,差异均有显著性意义(P0.05) 2)CK19表达与NPC分化有关:在中分化鼻咽癌中CK19阳性率为46.2%(6/13),低分化鼻咽中阳性率为81.6%(40/49),两者之间差异有显著性意义(P0.05)。 3)CK19表达与NPC原发瘤大小有关:CK19表达在无侵犯组的阳性率为20.0%(1/5),而在有周围侵犯组中的阳性率为78.9%(45/57),两者之间有显著性差异(P0.05)。 4)CK19表达与NPC淋巴结转移有关:CK19表达在无淋巴结转移的NPC中阳性率为11.1%(1/9),而在有淋巴结转移的NPC中为84.9%(45/53),两者之间有显著性差异。 5)CK19表达与NPC远处结转移有关:CK19在无远处转移的NPC中阳性率为71.4%(40/56),在有远处转移的NPC中为100%(6/6),两者之间有显著性差异(P0.05)。 6)CK19蛋白表达与NPC临床分期有关:Ⅰ+Ⅱ期NPC组织中CK19表达的阳性率为45.5%(5/11),而在Ⅲ+Ⅳ期的NPC中CK19表达的阳性率为78.8%(41/52),两者之间有显著性差异(P0.05)。 7)CK19蛋白表达与性别、年龄无关(P0.05) 8) Western blot结果显示,CK19在低分化NPC细胞系CNE2中的表达水平较高分化NPC细胞系CNE1明显上调;在高转移NPC细胞系5-8F中的表达水平较低转移NPC细胞系6-10B明显上调(P0.01),说明CK19表达与NPC分化及转移有关。 结论 1)鼻咽癌组织中CK19表达在淋巴结转移性鼻咽癌高于原发灶,在鼻咽癌中高于慢性鼻咽炎;分化差和高转移潜能的的鼻咽癌细胞中CK19表达高于分化好和低转移潜能的鼻咽癌细胞株,CK19可作为鼻咽癌分化和转移的分子标记物。 2)CK19表达与原发瘤大小、分化程度、淋巴结转移和远处转移、临床分期有关。与年龄、性别无关。
[Abstract]:Objective to study the expression of keratin 19 or Keratin 19 (CK19) in nasopharyngeal carcinoma (NPC) tissues and cells, and to analyze the relationship between the expression and clinicopathological features of nasopharyngeal carcinoma (NPC). Methods Sixty-two cases of nasopharyngeal carcinoma with lymph node metastasis and 16 cases of chronic nasopharyngitis (as normal nasopharyngeal mucosa control group) were confirmed by pathology. Immunohistochemical S-P method was used to detect the expression of CK19. The relationship between the expression level and clinicopathological features of nasopharyngeal carcinoma was analyzed, and the expression of CK19 protein in NPC cell lines with different differentiation and metastasis potential was detected by western blot. Results. 1) Immunohistochemical results showed that CK19 was mainly expressed in cytoplasm and membrane. The positive rate of CK19 in nasopharyngeal carcinoma was 74.2 / 66 / 62; the positive rate of lymph node metastatic nasopharyngeal carcinoma was 88.2 / 15 / 17; the positive rate of CK19 in normal nasopharyngeal epithelium in control group was 12.5% and the positive rate of CK19 in lymph node metastatic nasopharyngeal carcinoma was higher than that in nasopharyngeal carcinoma specimen, and was higher in nasopharyngeal carcinoma than in normal control group. The difference was significant (P0.05). 2the expression of CK19 was related to the differentiation of NPC: the positive rate of CK19 in moderately differentiated nasopharyngeal carcinoma was 46.2 / 13, and the positive rate in poorly differentiated nasopharynx was 81.6 / 49. There was a significant difference between the two groups (P 0.05). 3The expression of NPC CK19 was related to the size of the primary tumor of NPC. The positive rate of the expression of NPC in the non-invasive group was 20.0 / 5%, while in the group with peripheral invasion, the positive rate was 78.9% / 57%. There was a significant difference between the two groups (P 0.05). (4) the positive rate of NPC CK19 expression in NPC without lymph node metastasis was 11. 1 / 9 / 9, and that in NPC with lymph node metastasis was 84. 9 / 53. There was a significant difference between the two groups. 5The expression of CK19 was related to the distant node metastasis of NPC. The positive rate of NPC CK19 in NPC without distant metastasis was 71. 4% / 56%, and in NPC with distant metastasis was 100 / 6%.
