鼻丘上进路额窦手术可行性影像学研究

【摘要】:目的:探讨鼻内镜下鼻丘上进路治疗额窦病变新术式的可行性。方法:将32例患者CT资料导入Mimics软件中进行三维重建,在矢状位上分别测量"腋窝"中点和中鼻甲前端颅底附着点至筛前动脉的距离,冠状位上测量中鼻甲垂直板与眶纸板之间的距离、鼻丘的高度。应用Sinuses TracheaⅠ软件重建额窦及其周围气房三维可视结构。结合CT和上述数据,尸头模拟手术操作。结果:1中鼻甲前端颅底附着点位于额窦口前缘或稍靠后。2中鼻甲前端颅底附着点至筛前动脉的距离:左侧(15.31±2.82)mm、右侧(15.39±3.53)mm;"腋窝"中点至筛前动脉的距离:左侧(22.23±2.78)mm、右侧(22.30±2.80)mm;中鼻甲垂直板与眶纸板之间的距离:左侧(7.61±1.34)mm、右侧(7.80±1.40)mm;鼻丘的高度:左侧(8.33±2.14)mm、右侧(8.00±2.57)mm。以上每组数据左右对比差异无统计学意义(均P0.05)。3三维可视结构显示中鼻甲前端颅底附着点紧邻额窦口,其外下与眶纸板之间、鼻丘气房之上或上部区域仅为气房结构。结论:鼻内镜下鼻丘上进路额窦手术具有可行性,适用于孤立性额窦病变,可根据病变特点变通手术方式处理复杂额窦、额隐窝病变。
