bFGF和μPA蛋白在鼻咽癌组织中的表达及临床意义

【摘要】：目的:应用免疫组织化学方法,对bFGF和uPA在不同TNM分期及临床分期鼻咽癌(Nasopharyngeal Carcinoma,NPC)组织中的表达情况进行检测,分析其与临床病理因素和患者治疗后生存时间的关系。探讨它们在鼻咽癌发生发展过程中的作用,为鼻咽癌的早期诊断、预后判断提供新的理论基础。 方法:1.应用SupervisionTM二步法分别检测具有完整临床资料155例NPC组织和20例鼻咽粘膜慢性炎组织中bFGF、uPA蛋白表达情况。2.采用SPSS13.0统计软件进行数据处理,以P0.05为显著性水准。用χ2检验分析bFGF和uPA蛋白表达与NPC患者临床病理特征之间的关系;Spearman等级相关分析法分析两者之间相关性;Kaplan-Meier法进行单因素生存分析,生存曲线比较用Log-Rank检验。 结果:1. bFGF和uPA蛋白在NPC组织中的阳性表达率分别为80.6%和79.4%,在对照组织中基本不表达,bFGF和uPA蛋白在NPC和鼻咽粘膜慢性炎组织中阳性表达率的差异有统计学意义(P0.01)。2. bFGF和uPA蛋白在NPC表达与性别、年龄均无相关性(P0.05)。3. bFGF和uPA蛋白阳性表达率在NPC临床TNM分期的T1、T2、T3、T4分别为:53.3%、75.9%、84.2%、96.6%和53.3%、66.7%、91.2%、93.1% ,两指标在各自T期总体比较均有非常显著统计学意义(均为P0.01);在N0、N1、N2、N3分别为77.8%、69.8%、80.9%、94.7%和70.4%、69.8%、82.3%、92.1%,两指标在各自N期总体比较均有显著统计学意义(均为P0.05)。在M0、M1分别为78.0%、95.7%和76.5%、95.7%,两指标两组之间比较均有显著统计学意义(均为P0.05)。4. bFGF和uPA蛋白阳性表达率在临床分期为Ⅰ~Ⅱ、Ⅲ、Ⅳa、Ⅳb期分别为43.3%、87.7%、88.9%、95.7%和43.3%、84.2%、88.9%、95.7%,两指标在各自临床分期总体比较均有非常显著统计学意义(均为P0.01)。5. bFGF和uPA蛋白在鼻咽癌表达呈明显正相关(r=0.557,P=0.000)。6.单因素生存分析提示, bFGF和uPA蛋白单独或联合表达阳性患者治疗后中位生存时间短于阴性患者。存在淋巴结转移、远处转移,TNM分期晚期的患者提示有不良预后。 结论:1.鼻咽癌组中bFGF和uPA蛋白阳性的表达率明显高于炎症组,提示两基因可能参与肿瘤发生发展过程中。2. bFGF和uPA蛋白表达与性别、年龄无关,而与NPC的TNM分期和临床分期均有非常密切的关系,表明bFGF和uPA是NPC的局部浸润转移、淋巴结转移和远处器官转移的重要参与因子。3. bFGF和uPA蛋白表达呈显著正相关,表明它们很可能为NPC浸润转移的共同促进因子。4. bFGF和uPA与NPC的预后有关,提示bFGF和uPA阳性表达的NPC患者生存期短,预后差;二者联合表达有降低NPC患者生存时间的危险。
[Abstract]:Objective: to detect the expression of bFGF and uPA in nasopharyngeal carcinoma (NPC) with different TNM stages and clinical stages by immunohistochemical method, and to analyze the relationship between the expression of uPA and clinicopathological factors and survival time after treatment.To explore their role in the development of nasopharyngeal carcinoma and provide a new theoretical basis for the early diagnosis and prognosis of nasopharyngeal carcinoma.Method 1: 1.The expression of bFGF UPA protein in 155 cases of NPC and 20 cases of chronic nasopharyngeal mucositis was detected by SupervisionTM two-step method.Using SPSS13.0 statistical software for data processing, P0.05 as the significant level.The relationship between the expression of bFGF and uPA protein and the clinicopathological characteristics of NPC patients was analyzed by 蠂 2 test. The correlation between them was analyzed by Kaplan-Meier method. The survival curve was compared with Log-Rank test.Results the positive expression rates of bFGF and uPA in NPC were 80.6% and 79.4%, respectively. There was significant difference in the positive expression rate of NPC and uPA between NPC and chronic nasopharyngeal mucositis. BFGFAnd uPA protein expression and sex in NPC,The positive expression rate of bFGF and uPA protein in clinical TNM stage of NPC was 75.90% and 84.2%, respectively.
