中度非增殖期糖尿病视网膜病变个体化激光光凝术的应用研究

【摘要】: 目的:探讨以FFA(眼底血管荧光造影)计算机图像信息为引导进行糖尿病视网膜病变个体化激光光凝术对视网膜功能的影响。 方法:采用随机对照方法,选取符合眼底激光治疗适应症的中度非增殖期糖尿病视网膜病变(DR)患者60例(110眼),根据激光治疗的方式不同分成两组,以FFA计算机信息为引导个体化治疗组(实验组)和传统FFA图片下行全视网膜光凝治疗组(对照组),每组30例,随访24周。主要观察指标:术后视野光敏度变化、黄斑水肿、中心视力、毛细血管无灌注区、微血管瘤及新生血管等。 结果:两组术后视网膜光敏度下降,实验组下降1.23±0.47 dB,对照组下降2.88±0.68 dB,下降程度试验组明显低于对照组(p0.05);与激光术前相比实验组差异无统计学意义(p0.05),对照组差异有统计学意义(p0.05)。随访期内持续性黄斑水肿8眼其中实验组3,对照组5眼;中心视力改变、毛细血管无灌注区、微血管瘤消退、新生血管等两组差异均无统计学意义(p0.05)。 结论:视网膜光凝术对视野造成一定程度损害;个体化激光治疗方案能有效达到视网膜光凝治疗目的,而视网膜功能损伤明显小于对照组;应用个体化激光治疗方案无明显增加早期糖尿病视网膜病变视力丧失和其他并发症的风险。
[Abstract]:Objective: to investigate the effect of individualized laser photocoagulation on retinal function in diabetic retinopathy guided by FFA (fundus fluorescein angiography) image information.Methods: sixty patients with moderate nonproliferative diabetic retinopathy (DRR) were randomly divided into two groups according to the different methods of laser therapy.The whole retinal photocoagulation therapy group (control group, 30 cases) was followed up for 24 weeks with FFA computer information as the guide of individualized treatment group (experimental group) and traditional FFA image as the whole retinal photocoagulation treatment group (control group, 30 cases).Main outcome measures: postoperative visual field changes, macular edema, central visual acuity, capillary no perfusion area, microangioma and neovascularization.Results: after operation, the degree of retinal Guang Min in the two groups decreased 1.23 卤0.47 dB in the experimental group and 2.88 卤0.68 dB in the control group, which was significantly lower in the experimental group than in the control group, but there was no significant difference between the experimental group and the control group (P 0.05).There were 3 eyes in the experimental group and 5 eyes in the control group during the follow-up period. There was no significant difference in central visual acuity, capillary perfusion area, microangioma regression and neovascularization between the two groups (P 0.05).Conclusion: retinal photocoagulation has a certain degree of damage to the visual field, individualized laser therapy can effectively achieve the purpose of retinal photocoagulation, and retinal function damage is obviously smaller than that of the control group.Individualized laser therapy did not significantly increase the risk of vision loss and other complications in early diabetic retinopathy.
