C反应蛋白和血同型半胱氨酸与年龄相关性黄斑变性的相关性研究

【摘要】: 目的:探讨外周静脉血中的一些标记物对年龄相关性黄斑变性(age-related macular degeneration,AMD)的意义。 方法:选取2009年9月至2010年4月在重庆医科大学附属第一医院眼科门诊就诊愿意进行研究的患者,进行简单调查,调查内容包括年龄,性别,现病史,既往史,联系方式等。通过裂隙灯检查,直接眼底镜,间接眼底镜,光学相干断层成像(optical coherence tomography,OCT),眼底荧光血管造影(fluorescein fundus angiography,FFA),眼底照相(fundus photography,FP)等检查,选择确诊为AMD的患者进入病例组(group A)。按病例对照研究方法,选取重庆医科大学附属第一医院眼科住院患者中年龄50岁以上,即将手术的单纯性老年性白内障患者,作为对照组(group C)。分别抽取空腹静脉血检测以下11个指标: C反应蛋白(C reactive protein ,CRP),凝血酶原时间(prothrombin time,PT),凝血酶原时间比值(prothrombin ratio ,PTR),国际标准化比值(international normalized ratio ,INR),活动度(HGD),活化的部分凝血活酶时间(activated partial thromboplastin time,APTT),凝血酶时间(thrombin time,TT),纤维蛋白原(fibrinogen,FBG),叶酸(folic acid,FA),维生素B12(vitaminB12,VB12),血同型半胱氨酸(homocysteine,tHCY)。对数据结果采用统计软件包SPSS14.0进行统计分析。对病例组和对照组各个指标比较采用两独立样本均数的t检验,以P0.05为有统计学意义。 结果: 14例AMD患者CRP用均值±标准差表示为7.47±11.02(mg/l),5例对照组为6.48±4.49(mg/l),病例组与对照组无显著性差异(P0.05)。PT:病例组11.46±1.41(sec),对照组11.46±1.41(sec),病例组与对照组无显著性差异(P0.05)。PTR:病例组0.93±0.02,对照组0.95±0.07。病例组与对照组无显著性差异(P0.05)。INR:病例组0.92±0.02,对照组0.95±0.07。病例组与对照组无显著性差异(P0.05)。HGD:病例组123.05±6.56,对照组116.46±22.78。病例组与对照组无明显差异(P0.05)。APTT:20.42±2.05(sec),对照组23.66±2.71(sec)。病例组与对照组有显著性差异(P0.05)。TT:病例组17.34±2.73(sec),对照组16.38±0.82(sec)。病例组与对照组无显著性差异(P0.05)。FBG:病例组3.04±0.55(g/L),对照组2.86±.652(g/L)。病例组与对照组无显著性差异(P0.05)。FA:病例组8.30±4.10(ng/ml),对照组16.77±3.88(ng/ml)。病例组比对照组FA值低,有显著性差异(P0.05)。VB12:病例组518.41±201.03(Pg/ml),对照组354.22±73.50(Pg/ml)。病例组与对照组无显著性差异(P0.05)。tHCY:病例组18.43±6.37(umol/l),对照组13.30±5.19(umol/l)。病例组与对照组无显著性差异(P0.05)。 结论: 1.本研究未发现CRP和tHCY对AMD的诊断有直接意义。2.AMD患者体内叶酸含量明显低于未患AMD的同龄人,叶酸缺乏可能是引起AMD发生的原因之一。3. AMD患者APTT明显短于未患AMD的同龄人,可能与其血液高凝状态有关。
[Abstract]:Objective: to investigate the significance of some markers in peripheral venous blood for age-related macular degenerationin age-related macular degeneration.Methods: from September 2009 to April 2010, the patients who were willing to study in the first affiliated Hospital of Chongqing Medical University were investigated, including age, sex, current medical history, past history, contact method and so on.Through slit lamp examination, direct fundus microscopy, indirect fundus microscopy, optical coherence tomography, fluorescein fundus angiographyFFAA, fundus photography, and so on, the patients diagnosed as AMD were selected to enter group A.According to the case-control study, simple senile cataract patients, aged over 50 years old and about to be operated on, were selected as the control group in the first affiliated Hospital of Chongqing Medical University.The following 11 indexes were detected: C-reactive protein C reactive protein, prothrombin time, prothrombin ratio, international normalized ratio, active partial thromboplastin.Activated partial thromboplastin, thrombin time, fibrinogen, folic acid, vitamin B12, homocysteine and homocysteine.The data were analyzed by SPSS14.0 software package.Case group and control group were compared with each other using the t test of the average of two independent samples, with P0.05 as the statistical significance.Results: the mean 卤standard deviation of CRP in 14 patients with AMD was 7.47 卤11.02 mg / L, which was 6.48 卤4.49 mg / L in the control group. There was no significant difference between the case group and the control group (P 0.05). There was no significant difference between the case group (11.46 卤1.41) and the control group (11.46 卤1.41). There was no significant difference between the case group and the control group (P 0.05 卤0.02, P = 0.93 卤0.02, control group, 0.95 卤0.07).There was no significant difference between the case group and the control group (P 0.05). INR: the case group was 0.92 卤0.02, the control group was 0.95 卤0.07.There was no significant difference between the case group and the control group (P 0.05). HGD was 123.05 卤6.56 in the case group and 116.46 卤22.78 in the control group.There was no significant difference between the case group and the control group (P 0.05N. APTT: 20.42 卤2.05) and the control group (23.66 卤2.71).There was significant difference between the case group and the control group (P 0.05). TT: the case group was 17.34 卤2.73 and the control group was 16.38 卤0.82.There was no significant difference between the case group and the control group (P 0.05). FBG: the case group was 3.04 卤0.55 g / L, the control group was 2.86 卤.652 g / L.There was no significant difference between the case group and the control group (P 0.05). FA: the case group was 8.30 卤4.10 ng / ml, and the control group was 16.77 卤3.88 ng / ml 路ml ~ (-1).The FA value in the case group was lower than that in the control group (P 0.05). T HCY: the case group was 18.43 卤6.37 渭 mol / L, the control group was 13.30 卤5.19 渭 mol / L.There was no significant difference between the case group and the control group (P 0.05).Conclusion: 1.In this study, we found no direct significance of CRP and tHCY in the diagnosis of AMD. 2. The level of folic acid in patients with CRP is significantly lower than that of their peers without AMD. Folic acid deficiency may be one of the causes of AMD.The APTT of AMD patients was significantly shorter than that of their peers without AMD, which may be related to their blood hypercoagulability.
