糖尿病视网膜病变新生血管生成与血清VEGF、ES、TSP关系研究

【摘要】:目的: 探讨糖尿病性视网膜病变(diabetic retinopathy,DR)患者血清VEGF、ES、 TSP水平及它们与眼底病变的关系,新生血管生成因子与抑制因子关系。 方法: 采用ELISA法检测51例DR患者(观察组)和10例无DR表现的糖尿病人(对照组)血清血管内皮生长因子(vascular endothelial growth factor,VEGF)、内皮抑素(endostatin,ES)及血小板反应蛋白(thrombospondin,TSP)水平,对比分析各指标及其与眼底病变的关系并探讨新生血管生成因子与抑制因子关系。 结果: 不同程度糖尿病性视网膜病变组之间VEGF有统计学意义(P0.01),且VEGF水平呈递增趋势;不同程度糖尿病性视网膜病变组之间ES有统计学意义(P0.01),ES水平在对照组与非增生性糖尿病视网膜病变(NPDR)组病人呈递增趋势,在增生性糖尿病视网膜病变(PDR)组却显著降低;TSP水平在不同程度糖尿病性视网膜病变组之间没有统计学意义(P0.05)。VEGF与眼底病变程度具有显著相关性(r=0.84,p0.05);对照组与非增生性糖尿病视网膜病变(NPDR)病人血清中ES与眼底病变程度具有显著相关性(r=0.82,P0.05);TSP与眼底病变程度无相关性(r=-0.06,p0.05)。对照组+NPDR病人血清VEGF与ES具有相关性(r=0.73,p0.05);增生性糖尿病视网膜病变(PDR)病人血清VEGF与ES之间无相关性(r=0.01,p0.05),VEGF与TSP之间无相关性(r=-0.13,p0.05),ES与TSP之间无相关性(r=0.01,p0.05)。 结论: 血清VEGF与ES水平在非增生性糖尿病视网膜病变病人中处于平衡状态,而在增生性糖尿病视网膜病变病人时VEGF与ES之间这种平衡状态的改变可能决定了增生性糖尿病视网膜病变时新生血管的发生,提示血管生成因子和血管抑制因子作用的失衡可能是增生性糖尿病视网膜病变新生血管形成的原因。
[Abstract]:Objective: To investigate the serum levels of VEGF ESand TSP in diabetic retinopathy and their relationship with fundus lesions and the relationship between neovascularization factor and suppressor factor. Methods: The relationship between neovascularization factor and suppressor factor was studied. Results: There was a significant difference in VEGF between different degree diabetic retinopathy group (P 0.01), and the level of VEGF showed an increasing trend. The levels of es in different degree diabetic retinopathy group were significantly higher than those in control group and non-proliferative diabetic retinopathy group. There was a significant correlation between serum es and the degree of fundus lesion in patients with diabetic retinopathy (NPDR). There was no correlation between tsp and the degree of fundus lesion. Conclusion: Serum levels of VEGF and es were balanced in patients with non-proliferative diabetic retinopathy. The change of the balance between VEGF and es in patients with proliferative diabetic retinopathy may determine the occurrence of neovascularization in proliferative diabetic retinopathy. The results suggest that the imbalance of angiogenesis and angiostatin may be the cause of angiogenesis in proliferative diabetic retinopathy.
