中耳胆脂瘤上皮中Livin、p73的表达与其发生关系研究

【摘要】: 目的:探讨凋亡抑制蛋白Livin及p73在中耳胆脂瘤上皮中发病机制的作用。方法:采用免疫组织化学技术SP法检测30例中耳胆脂瘤上皮及16例外耳道正常皮肤组织中凋亡抑制蛋白Livin及p73的表达,并结合患者临床特征、中耳胆脂瘤对听小骨骨质破坏程度进行分析。结果:凋亡抑制蛋白Livin表达于细胞核及部分细胞质,分布于胆脂瘤上皮全层,以基底层为主;p73表达于胞浆,分布于胆脂瘤上皮棘层和颗粒层;上述两指标在中耳胆脂瘤上皮中的阳性表达率分别为76.7%、40%,凋亡抑制蛋白Livin在胆脂瘤上皮中的表达高于外耳道正常皮肤,差异有统计学意义(P0.05);p73在胆脂瘤上皮中的表达与外耳道正常皮肤之间无统计学意义(P0.05);听小骨骨质破坏程度、性别、民族、胆脂瘤侵犯范围与上述指标的表达差异无统计学意义(P0.05);胆脂瘤上皮中凋亡抑制蛋白Livin的表达与p73的表达呈正相关(r=0.411,P0.05)。结论:凋亡抑制蛋白Livin在中耳胆脂瘤的异常表达可能在胆脂瘤的发生、发展过程中起相应作用,可能参与胆脂瘤上皮的凋亡调控过程,而p73在中耳胆脂瘤的发生和发展中的作用尚难确定。胆脂瘤上皮的凋亡与增殖与听小骨破坏程度尚无直接关系。
[Abstract]:Objective: to investigate the role of Livin and p73 in the pathogenesis of middle ear cholesteatoma. Methods: immunohistochemical SP method was used to detect the expression of Livin and p73 in 30 cases of middle ear cholesteatoma and 16 cases of normal skin of external auditory canal. Middle ear cholesteatoma was used to analyze the degree of destruction of osseous auditory bone. Results: Livin was expressed in the nucleus and some cytoplasm, distributed in the whole layer of cholesteatoma epithelium, mainly in the cytoplasm of the basal layer, in the spinous layer and granular layer of the epithelium of cholesteatoma. The positive expression rates of these two markers in the epithelium of middle ear cholesteatoma were 76. 7% and 40%, respectively. The expression of apoptosis inhibitor protein Livin in the epithelium of cholesteatoma was higher than that in normal skin of external auditory canal. There was no significant difference between the expression of P0.05P73 in the epithelium of cholesteatoma and the normal skin of external auditory canal. There was no significant difference between the extent of cholesteatoma invasion and the expression of the above indexes. The expression of apoptosis inhibitor protein (Livin) in cholesteatoma epithelium was positively correlated with the expression of p73. Conclusion: the abnormal expression of apoptosis inhibitor protein Livin in middle ear cholesteatoma may play a corresponding role in the occurrence and development of cholesteatoma, and may participate in the process of apoptosis regulation of cholesteatoma epithelium. The role of p73 in the development and development of middle ear cholesteatoma is uncertain. There is no direct relationship between apoptosis and proliferation of cholesteatoma epithelium and the degree of destruction of auditory bone.
