高度近视眼视盘旁Zinn-Haller动脉环荧光血管造影特征分析

【摘要】:目的探讨高度近视患者视盘旁Zinn-Haller动脉环的特征。方法回顾分析高度近视眼患者198例(396眼)的眼底彩色照片及眼底荧光血管造影结果,筛选具有Zinn-Haller动脉环的患者并结合视力、屈光度等进行总结。结果 24例患者眼底荧光血管造影显示:Zinn-Haller环以视盘为中心,位于视盘颞侧视盘边缘至新月形萎缩弧之间。所有患者均未见完整的Zinn-Haller环。眼底荧光血管造影动脉前期:随着脉络膜背景荧光的出现,即可见视盘颞侧新月形萎缩弧内部出现Zinn-Haller动脉环的显影;动脉期:当视网膜动脉开始出现动脉前锋时,Zinn-Haller环的显影已经完成,其最明显的部分即位于视盘颞侧萎缩弧内,此期可清楚分析Zinn-Haller环与视盘旁血管的位置关系。眼底荧光血管造影静脉期可见Zinn-Haller环完全显影,其走行形态非规则环形,可见迂曲转角,部分患者甚至呈锯齿状走行。结论 Zinn-Haller环在不同患者中存在解剖位置及形态分布上的差异,这些差异可能成为缺血性视神经病变、青光眼等视神经病变的解剖学及生理学基础。
[Abstract]:Objective to investigate the characteristics of Zinn-Haller arterial ring in patients with high myopia. Methods the fundus color photographs and fundus fluorescein angiography (FFA) of 198 patients with high myopia were retrospectively analyzed. The patients with Zinn-Haller arterial ring were selected and summarized with visual acuity and diopter. Results the fundus fluorescein angiography of 24 patients showed that the circumferential circumference of the 1: Zinn-Haller circle was centered on the optic disc between the edge of the temporal lateral disc of the optic disc and the crescent atrophy arc. No complete Zinn-Haller ring was found in all patients. In the prearterial phase of fundus fluorescein angiography (FFA): with the appearance of choroidal background fluorescence, the Zinn-Haller arterial ring could be seen in the temporal atrophic arc of the optic disc. Arterial phase: the development of Zinn-Haller ring was completed when the front of retinal artery began to appear. The most obvious part was located in the temporal atrophy arc of optic disc. The relationship between the Zinn-Haller ring and the para-optic vessels could be clearly analyzed at this stage. In fundus fluorescein angiography (FFA), the Zinn-Haller loop was completely developed, with irregular circular pattern and tortuous turning angle, and some patients even showed sawtooth pattern in the vein phase of fundus fluorescein angiography (FFA). Conclusion there are differences in anatomical location and morphological distribution of Zinn-Haller ring in different patients. These differences may be the anatomical and physiological basis of ischemic optic neuropathy, glaucoma and other optic neuropathy.
