微创玻璃体切割联合超声乳化术治疗增生性糖尿病视网膜病变的疗效观察

【摘要】：目的探讨微创玻璃体切割联合超声乳化术治疗增生性糖尿病视网膜病变(proliferative diabetic retinopathy,PDR)的疗效。方法选择2011年1月至2012年12月我院收治的PDR患者120例(142眼),均给予玻璃体切割联合超声乳化吸出人工晶状体植入术治疗,根据手术切口不同随机分为微创切口的观察组(60例70眼)和常规切口的对照组(60例72眼)。术中观察并记录两组患者的生命体征,术后观察两组患者的并发症情况。术前和术后1周、2周、3周和4周检测两组患者的视力和角膜散光情况。结果两组术中收缩压、舒张压及心率比较,差异均无统计学意义(均为P0.05);观察组术后角膜水肿(8.33%)、角膜混浊(3.33%)、虹膜粘连(1.67%)发生率均明显低于对照组(13.33%、6.67%、3.33%),差异均有统计学意义(均为P0.05)。观察组、对照组术后1周、2周、3周和4周视力较术前显著增加,角膜散光较术前显著降低,差异均有统计学意义(均为P0.05);观察组术后1周、2周、3周和4周视力高于对照组,角膜散光低于对照组,差异均有统计学意义(均为P0.05)。结论微创玻璃体切割联合超声乳化术治疗PDR能够促进视力恢复,减少并发症发生。
[Abstract]:Objective to evaluate the efficacy of minimally invasive vitrectomy combined with phacoemulsification in the treatment of proliferative diabetic retinopathy (PDR) in patients with proliferative diabetic retinopathy. Methods from January 2011 to December 2012, 120 patients with PDR were treated with vitrectomy combined with phacoemulsification and intraocular lens implantation. According to the surgical incision, the patients were randomly divided into observation group (n = 60, n = 70) and control group (n = 60, n = 72). The vital signs of both groups were observed and recorded during operation and the complications were observed after operation. The visual acuity and corneal astigmatism were measured before and 1 week, 2 weeks, 3 weeks and 4 weeks after operation. Results there was no significant difference in systolic blood pressure, diastolic blood pressure and heart rate between the two groups (all P 0.05). The incidence of corneal edema, corneal opacification and iris adhesion in the observation group was significantly lower than that in the control group (P < 0.05). In the observation group, the visual acuity and corneal astigmatism in the control group increased significantly 1 week and 2 weeks after operation, and the corneal astigmatism decreased significantly (all P 0.05), and the visual acuity of the observation group was higher than that of the control group at 1 week, 2 weeks, 3 weeks and 4 weeks after operation, and the visual acuity of the control group was significantly higher than that of the control group. Corneal astigmatism was significantly lower than that in control group (P 0.05). Conclusion minimally invasive vitrectomy combined with phacoemulsification in the treatment of PDR can promote visual recovery and reduce complications.
