低温等离子扁桃体被膜内切除术临床研究

【摘要】：目的：探索扁桃体被膜内切除术的手术方法，减轻手术创伤，降低术后并发症的发生，为临床实践提供一种可靠的新型手术方式。方法：选取2012年12月至2013年12月遵义医学院附属医院耳鼻喉科以扁桃体肥大收入院，拟行扁桃体切除术的住院病人，40例，其中，，男25例，女15例，随机分为2组：低温等离子扁桃体被膜内切除术组（简称被膜内切除组）20例及低温等离子扁桃体被膜外切除术组（简称被膜外切除组）20例，比较其手术时间、术中出血量、术后疼痛、术后并发症发生率，白膜脱落时间。结果：术中血量被膜内切除组为（6.20±1.36）ml，被膜外切除组为（9.50±2.04）ml；术后疼痛（经疼痛量表评分）：术后6小时：被膜内切除组为（2.50±0.59）,被膜外切除组为（3.80±0.70），术后第一天被膜内切除组为（2.30±0.46）,被膜外切除组为（3.30±0.67），术后第二天被膜内切除组为（1.55±0.50）,被膜外切除组为（2.30±0.57）；术后出血发生率（继发性出血）：被膜内切除组未发生出血，被膜外切除组为10%；所得数据采用SPSS17.0软件包进行统计学分析后有统计学意义（P0.5）；术后白膜脱落时间：被膜内切除组为（7.15±0.85）天；被膜外切除组为：（7.15±1.09）天；伤口感染发生率：被膜内切除组为10%，被膜外切除组为：25%；手术时间：被膜内切除组为（18.4±2.42）分钟，被膜外切除组为（17.7±2.00）分钟，所得数据采用SPSS17.0软件包进行统计学分析后无统计学意义（P0.5）。结论：应用低温等离子行扁桃体被膜内切除术是一种崭新的手术方式，具有创伤小、恢复快、疼痛轻且并发症发生率低等优点，通过对低温等离子扁桃体被膜内切除术的研究，为扁桃体切除术的临床工作提供一种更佳的手术方式的选择，值得临床推广。
[Abstract]:Objective: to explore the surgical method of tonsillectomy, to reduce surgical trauma and postoperative complications, and to provide a new and reliable surgical method for clinical practice. Methods: from December 2012 to December 2013, 40 inpatients (25 males and 15 females) with tonsillectomy were enrolled in the Department of Otorhinolaryngology, affiliated Hospital of Zunyi Medical College. Two groups were randomly divided into two groups: low temperature plasma tonsillectomy group (20 cases) and hypothermic plasma tonsillectomy group (20 cases). Postoperative pain, incidence of postoperative complications, white film fall off time. Results: intraoperative blood volume was 6.20 卤1.36ml in the Tunica resection group and 9.50 卤2.04ml in the extramembranous resection group, and postoperative pain was 2.50 卤0.59m in the intrathecal resection group, 3.80 卤0.70 in the extramembranous resection group and 3.80 卤0.70 in the first day after operation. The incidence of postoperative hemorrhage was 3.30 卤0.67 in the extramembranous resection group, 1.55 卤0.50 in the intramembranous resection group and 2.30 卤0.57 in the extramembranous resection group on the second day after operation. There was significant difference in the data obtained by SPSS17.0 software package after statistical analysis, the time of white membrane shedding after operation was 7.15 卤0.85 days in the intrathecal resection group and 7.15 卤1.09 days in the extramembranous resection group. The incidence of wound infection in the intramembranous resection group was 10 and that in the extramembranous resection group was 25: 25.The operative time was 18.4 卤2.42 minutes in the intramembranous resection group and 17.7 卤2.00 minutes in the extramembranous resection group. There was no significant difference between the two groups after statistical analysis by SPSS17.0 software package. Conclusion: Intra-membrane resection of tonsillectomy with hypothermic plasma is a new method with the advantages of small trauma, quick recovery, mild pain and low incidence of complications. It provides a better choice for tonsillectomy and is worth popularizing.
