【摘要】:目的:研究凋亡抑制基因Survivin, Bcl-2, Pml mRNA在鼻咽癌中的表达,通过采用实时荧光定量PCR方法检测其在鼻咽癌组织中和外周血及慢性鼻咽炎组织、鼻咽癌放疗后外周血中的表达,来探讨Survivin, Bcl-2, Pml中单个基因及相互作用下与鼻咽癌的临床分期及预后的相关性。 材料和方法:采用实时荧光定量PCR的方法检测60例就诊于昆明医学院第一附属医院耳鼻喉科,鼻咽部取材并通过病理学确诊为鼻咽癌的50例的活检组织和外周血,慢性鼻咽炎多10例活组织,检测Survivin, Bcl-2, Pml mRNA的表达,观察三个基因在鼻咽癌和慢性鼻咽炎的活组织中的表达,观察他们之间的相关性。并追踪随访这些患者通过放疗后的外周血中Survivin, Bcl-2, Pml的表达水平,来监测Survivin, Bcl-2, Pml三个基因在鼻咽癌中的表达的相关性,观察其与鼻咽癌预后的相关性。 结果:本研究中,Survivin, Bcl-2, Pml mRNA基因在鼻咽癌组织中的表达较慢性鼻咽炎组织中高,两者差异有统计学意义(P0.01)。放疗前的外周血中Survivin, Bcl-2, Pml mRNA基因相对表达量与放疗后之间有显著的差异(P0.01),放疗前组织中Survivin, Bcl-2, Pml mRNA基因相对表达量于放疗后外周血之间有显著差异(P0.01),Ⅲ、Ⅳ期中Survivin, Bcl-2, Pml mRNA的阳性检出率比工、II期患者之间增高,有颈淋巴结转移的鼻咽癌中,Survivin, Bcl-2, Pml mRNA的阳性检出率比无颈淋巴结转移者之间增高,提示Survivin, Bcl-2, Pml基因的表达与鼻咽癌患者的临床病程及预后有显著关系。而Survivin, Bcl-2, Pml mRNA基因的表达与鼻咽癌患者的年龄、性别无关(P0.05)。 结论:通过检测发现Survivin, Bcl-2,Pml的mRNA在鼻咽癌中的表达量与鼻咽癌的临床分期及病理分期呈正相关,并与鼻咽癌原发病灶侵犯程度、淋巴结浸润、远处转移和TNMI临Ⅰ床分期有相关性,而与患者的年龄、性别、无相关性,因此,Survivin, Bcl-2, Pml mRNA可作为研究鼻咽癌发生、发展过程及临床分期、预后判断的标记。
[Abstract]:Objective: to study the expression of survivin, Bcl-2 and Pml mRNA in nasopharyngeal carcinoma (NPC), and to detect the expression of survivin, Bcl-2 and Pml mRNA in nasopharyngeal carcinoma (NPC) tissues, peripheral blood and chronic nasopharyngitis by real-time fluorescence quantitative PCR. To investigate the relationship between the clinical stage and prognosis of nasopharyngeal carcinoma (NPC) by single gene and interaction in survivin, Bcl-2, Pml. Materials and methods: Real-time fluorescence quantitative PCR was used to detect the biopsy tissues and peripheral blood of 60 cases of nasopharyngeal carcinoma (NPC) from the otolaryngology department of the first affiliated hospital of Kunming Medical College. The expression of survivin, Bcl-2 and Pml mRNA was detected in 10 cases of chronic nasopharyngitis, and the expression of three genes in the living tissues of nasopharyngeal carcinoma and chronic nasopharyngitis was observed, and the correlation between them was observed. The expression levels of survivin, Bcl-2 and Pml in peripheral blood of these patients were followed up after radiotherapy to monitor the expression of survivin, Bcl-2 and Pml in nasopharyngeal carcinoma, and to observe the correlation between survivin, Bcl-2 and Pml in the prognosis of nasopharyngeal carcinoma. Results: the expression of survivin, Bcl-2 and Pml mRNA in nasopharyngeal carcinoma was higher than that in chronic nasopharyngitis, and the difference was statistically significant (P 0.01). There were significant differences in the relative expression of survivin, Bcl-2, Pml mRNA gene in peripheral blood before radiotherapy and after radiotherapy.
