【摘要】：目的：通过检测角膜、虹膜及房水中抗体浓度的变化,探讨体液免疫应答与白内障术后前房炎症反应的关系,并进一步探讨α晶状体蛋白抗体与白内障术后前房炎症反应的相关性。方法：家兔40只,随机选取8只,作为对照组(摘除正常眼球),其余家兔行超声乳化术,术后通过分层抽样分为A组(术后1d摘除眼球)、B组(术后1周摘除眼球)、C组(术后2周摘除眼球)及D组(术后1个月摘除眼球),每组8只,并分别于术前、术后1d、1周、2周、1个月通过裂隙灯观察房水闪辉及角膜水肿情况,免疫荧光法检测角膜及虹膜中IgG抗体平均积分光密度值(MOD)。健康家兔40只,均行超声乳化术,分别于术前、术后1天、1周、2周、1个月抽取家兔房水样品,通过间接酶联免疫吸附试验(Indirect enzyme-linked immunosorbent assay,Indirect ELISA)检测房水中α晶状体蛋白抗体,用免疫比浊法检测房水中IgG抗体含量。结果：①平均积分光密度(MOD)：对照组：角膜：388.00±11.90,虹膜：1.04±0.23；A组：角膜：470.60±105.07,虹膜：177.70±61.66；B组：角膜：402.40±19.59,虹膜：42.41±3.42；C组：410.70±19.70,虹膜：5.15±0.61；D组：角膜：396.02±18.425,虹膜：1.34±0.37。各实验组虹膜中IgG积分光密度均数均较对照组升高,但D组与对照组统计分析结果无显著差异性；各组家兔角膜中抗体与对照组均无统计学差异,但白内障术后角膜中抗体MOD值的均数均较对照组升高,尤其是在白内障术后早期重度水肿的家兔角膜中抗体的免疫荧光定量分析结果显著升高。②房水中IgG及α晶状体蛋白抗体检测：房水中IgG抗体(mg/dl):术前：3.46±1.04,术后1天：8.24±2.56,术后1周：4.68±1.32,术后2周：4.22±1.08,术后1月：3.68±1.02。白内障术后房水中IgG抗体含量增高,至一个月时接近正常水平。α晶状体蛋白抗体检测：白内障术后1d,6只家兔房水中检测到α晶状体蛋白抗体,均角膜重度水肿,房水闪辉显著。术后1周仅1只家兔房水闪辉仍较重,a晶状体蛋白抗体阳性,且α晶状体蛋白抗体与房水中IgG抗体含量有相关性。结论;白内障术后,体液免疫应答与房水闪辉及角膜水肿有关；针对α晶状体蛋白的体液免疫应答只在白内障术后重度前房炎症反应中发挥作用。
[Abstract]:Objective: to investigate the relationship between humoral immune response and anterior chamber inflammation after cataract surgery by detecting the concentration of antibodies in cornea, iris and aqueous humor. Furthermore, the relationship between 伪 -crystallin antibody and anterior chamber inflammation after cataract surgery was investigated. Methods: 40 rabbits were randomly selected as control group. The patients were divided into two groups by stratified sampling: group A (1 day after operation) and group C (removing eyeball at 1 week) and group D (1 month after operation with 8 eyes in each group). The glare of aqueous humor and corneal edema were observed by slit lamp and the mean integral optical density of IgG antibody in cornea and iris was detected by immunofluorescence. 40 healthy rabbits were treated with phacoemulsification. The samples of aqueous humor were collected from rabbits before operation, 1 day after operation and 1 week after operation. The antibodies of 伪 -crystallin in aqueous humor were detected by indirect enzyme-linked immunosorbent assayay assay (Elisa). The content of IgG antibody in aqueous humor was detected by immunoturbidimetric method. Results: the average integral optical density (MMD) of 1: 1: control group: cornea: 1: 388.00 卤11.90, iris: 1. 04 卤0. 23A: group A: cornea: 470.60 卤105.07; iris: group B: 1: 402.40 卤19. 59; iris: 42.41 卤3. 422: C group: 410.70 卤19.70; iris: 5. 15 卤0. 61: d group: cornea: 396.02 卤18.425; iris: 1. 34 卤0. 377. The mean value of integrated optical density of IgG in the iris of each experimental group was higher than that of the control group, but there was no significant difference between group D and control group, and there was no significant difference in antibody in cornea between group D and control group. However, the mean value of MOD in cornea after cataract surgery was higher than that in control group. Especially in rabbits with severe edema after cataract surgery, the results of immunofluorescence quantitative analysis of antibodies in cornea increased significantly. 2 the detection of IgG and 伪 -crystallin antibodies in aqueous humor: IgG antibody in aqueous humor was measured by mg / dll: before operation: 3. 46 卤1. 04, 1: 1 after operation. Day: 8.24 卤2.56, 1 week: 1: 4.68 卤1.32, 2 weeks after operation: 4. 22 卤1. 08, 1 month after operation: 3. 68 卤1. 02. The level of IgG antibody in aqueous humor after cataract increased to a normal level at one month. 伪 -crystallin antibody was detected in aqueous humor of 6 rabbits 1 day after cataract operation, all of them had severe corneal edema, and aqueous humor flashes were remarkable. 1 week after operation, only 1 rabbit aqueous humor was still positive for severe 伪 -crystallin antibody, and there was a correlation between 伪 -crystallin antibody and IgG antibody content in aqueous humor. Conclusion: the humoral immune response is related to aqueous flash and corneal edema after cataract, and the humoral immune response to 伪 -crystallin only plays a role in severe anterior chamber inflammation after cataract surgery.
