有晶状体眼后房型人工晶状体植入术治疗成人近视性屈光参差性弱视

【摘要】：目的研究有晶状体眼后房型人工晶状体又称可植入式接触镜(implantable contact lens,ICL)植入术治疗成人近视性屈光参差性弱视的效果和安全性。方法2010年9月到2012年9月在汉口爱尔眼科医院行ICL植入术的伴有弱视的高度近视性屈光参差患者共11例,随访6个月以上。记录患者术前、术后裸眼视力、最佳矫正视力、眼压、角膜内皮细胞计数以及术后的Titmus立体视情况。结果术前患者弱视眼平均屈光度为13.55 D(等效球镜),术后为0.33 D。术前最佳矫正视力为0.50±0.18,术后1周裸眼视力为0.72±0.23,末次随访时裸眼视力进一步提升到0.78±0.22,较术前最佳矫正视力平均提高2行,差异有显著统计学意义(t=-9.87,P0.001)。术前角膜内皮细胞计数为(2678±155)个·mm-2,术后1周为(2466±214)个·mm-2,末次随访时为(2410±142)个·mm-2。术后Titmus立体视检查2例斜视患者没有立体视,其他患者均有立体视,立体视为157″±102″。所有术眼术中、术后均未见严重并发症发生。结论ICL植入术是治疗成人近视性屈光参差性弱视安全有效的方法,其能明显提高患者的视功能,改善患者的生活质量。
[Abstract]:Objective to study the efficacy and safety of implantation of posterior chamber intraocular lens (IOL) in the treatment of myopic anisometropia amblyopia in adults. Methods from September 2010 to September 2012, 11 patients with high myopic anisometropia with amblyopia underwent ICL implantation in Hankou Eyre Eye Hospital. Preoperative and postoperative naked visual acuity, best corrected visual acuity, intraocular pressure, corneal endothelial cell count and Titmus stereopsis were recorded. Results the mean diopter of amblyopic eyes was 13.55 D before operation and 0.33 D after operation. The preoperative best corrected visual acuity (BCVA) was 0.50 卤0.18, and the uncorrected visual acuity was 0.72 卤0.23 at 1 week postoperatively. At the last follow-up, the uncorrected visual acuity was further improved to 0.78 卤0.22, which was 2 lines higher than that of the preoperative best corrected visual acuity (BCVA). The difference was statistically significant. The corneal endothelial cell count was 2678 卤155mm-2before operation, 2466 卤214mm-2at 1 week postoperatively, and 2410 卤142mm-2at the last follow-up. After operation, 2 patients with strabismus did not have stereopsis, and all other patients had stereopsis, which was considered as 157 "卤102". There were no serious complications in all eyes. Conclusion ICL implantation is a safe and effective method for the treatment of adult myopic anisometropia amblyopia. It can obviously improve the visual function and the quality of life of the patients.
